彩色多普勒超声联合血清肿瘤标志物诊断卵巢占位的效能分析

摘　　要：目的探讨彩色多普勒超声联合血清癌胚抗原(CEA)、人附睾蛋白4(HE4)、糖类抗原125(CA125)检测卵巢良恶性占位的诊断效能。方法选取2018年1月-2019年6月本院收治的卵巢良恶性占位患者180例,所有患者均接受彩色多普勒超声检查。采用酶联免疫吸附试验(ELISA)检测对血清CEA、HE4、CA125水平进行检测,比较卵巢良、恶性占位患者的彩色多普勒超声血流指标及血清CEA、HE4、CA125水平。以病理结果为标准,计算彩色多普勒超声及血清CEA、HE4、CA125单独和联合检测诊断卵巢良恶性占位的灵敏度和特异性等。结果卵巢癌患者的RI、PI值均明显低于卵巢良性肿瘤患者,超声评分及血清CEA、CA125、HE4水平均明显高于卵巢良性肿瘤患者(P<0.01);彩色多普勒超声联合血清CEA、CA125、HE4检测诊断卵巢良恶性占位的效能最高,灵敏度、特异度、阳性预测值、阴性预测值分别为96.00%、93.75%、95.05%、94.94%。结论彩色多普勒超声联合血清CEA、CA125、HE4检测较单独检测诊断卵巢良恶性占位的性能明显提高,值得临床推广。Objective To investigate the diagnostic efficacy of color Doppler ultrasound combined with serum carcinoembryonic antigen(CEA),human epididymal protein 4(HE4)and carbohydrate antigen 125(CA125)in benign and malignant ovarian mass.Methods A total of 180 benign and malignant ovarian occupying patients admitted to our hospital from January 2018 to June 2019 were selected,and all of whom underwent color Doppler ultrasonography.Serum CEA,HE4 and CA125 levels were detected by enzyme-linked immunosorbent assay(ELISA),and color Doppler ultrasound blood flow indexes and serum CEA,HE4 and CA125 levels in patients with benign and malignant ovarian mass were compared.The sensitivity,specificity,positive predictive value and negative predictive value of color Doppler ultrasonography and serum CEA,HE4 and CA125 alone and in combination were calculated for the diagnosis of benign and malignant ovarian occupying.Results The RI and PI values of ovarian cancer patients were significantly lower than those of patients with benign ovarian tumors,and the ultrasonic scores and serum CEA,CA125 and HE4 levels were significantly higher than those of patients with benign ovarian tumors,with the differences statistically significant(P<0.01).Color Doppler ultrasound combined with serum CEA,CA125 and HE4 had the highest detection efficiency for benign and malignant ovarian occupying,with sensitivity,specificity,positive predictive value and negative predictive value of 96.00%,93.75%,95.05%and 94.94%,respectively.Conclusion Color Doppler ultrasound combined with serum CEA,CA125 and HE4 can significantly improve the diagnostic efficiency of benign and malignant ovarian occupying,which has important value for the early diagnosis of ovarian cancer.
