多囊性卵巢综合征患者AMH水平与性激素水平及妊娠结局的关系

摘　　要：目的研究多囊性卵巢综合征(PCOS)患者抗苗勒管激素(AMH)水平与性激素水平及妊娠结局的关系。方法95例采用双压降调节长方案治疗的PCOS患者,于月经第3天(口服达英-35前)采集空腹静脉血检测AMH、性激素水平;根据AMH水平分为低、中及高AMH组,比较3组患者的基础促卵泡激素(FSH)、基础促黄体生成素(LH)、FSH/LH、雌二醇(E2)、外源性促性腺激素(Gn)及睾丸酮(T)水平和可移植胚胎数、临床妊娠率等资料;采用多元线性回归分析PCOS患者AMH水平与FSH、LH、FSH/LH、E2、外源性促性腺激素(Gn)用量、可移植胚胎数的相关性。结果95例PCOS患者中低AMH占34.74%、中AMH占32.63%、高AMH占32.63%,3组患者基础FSH、基础LH、FSH/LH、E 2、Gn用量及可移植胚胎数比较,差异均有统计学意义(P<0.05),其中高AMH组基础FSH、基础LH低于中AMH组,FSH/LH低于低AMH组,E2低于低AMH组和中AMH组,而T水平比较差异无统计学意义(P>0.05);PCOS患者血清AMH水平与基础FSH、基础LH及E 2呈负相关(r=-0.551、-0.409、-0.229,P<0.05)。结论PCOS患者AMH水平与FSH、LH、E 2明显相关。Objective To study the relationship of anti-Müllerian hormone(AMH)level with sex hormone levels and pregnancy outcomes in patients with polycystic ovary syndrome(PCOS).Methods Totally 95 patients with PCOS treated with the long protocol of dual-pressure down-regulation at the assisted reproductive center of the hospital were selected.Fasting venous blood was collected from the patients on the third day of menstruation(before oral administration of Diane-35)to measure the levels of AMH and sex hormones.Sex hormone levels,clinical pregnancy rates,and other data of patients with different AMH levels were compared.Multiple linear regression analysis was performed to analyze the relationship of AMH level with sex hormone levels and pregnancy outcomes in patients with PCOS.Results In the 95 patients with PCOS,patients with low AMH levels(low AMH group),medium AMH levels(medium AMH group),and high AMH levels(high AMH group)accounted for 34.74%,32.63%,and 32.63%,respectively.There were statistically significant differences in basic follicle-stimulating hormone(FSH),basic luteinizing hormone(LH),FSH/LH,estradiol(E 2),dosage of exogenous gonadotropin(Gn),and the number of transplantable embryos among the three groups(P<0.05),but there were no statistically significant differences in T levels(P>0.05).The basic FSH and basic LH of high AMH group were lower than those of medium AMH group,FSH/LH was lower than that of low AMH group,and E 2 was lower than those of low AMH group and medium AMH group(P<0.05).Serum AMH levels in patients with PCOS were significantly negatively correlated with basic FSH,basic LH,and E 2(r=-0.551,-0.409,-0.229).Conclusion AMH levels of patients with PCOS are significantly correlated with FSH,LH,and E 2.
