摘　　要：目的探讨鼻腔鼻窦恶性黑色素瘤(SNMM)的预后及其影响因素。方法采用回顾性分析方法,收集郑州大学第一附属医院2011年10月至2019年10月临床资料完整的52例SNMM患者作为研究对象,运用Kaplan-Meier法及Cox回归模型进行单因素和多因素生存分析,得到影响患者预后的因素。结果SNMM患者中,男28例,女24例,平均年龄为62.3岁,年龄6~82岁,生存时间为5~96个月,5年生存率为23.1%,中位生存期为29.3个月。经Cox回归模型分析发现,临床分期、Ki-67表达水平、辅助治疗是影响SNMM患者5年生存率的独立因素。结论SNMM侵袭性强,转移率高,应该术后定期复查,加强随访。免疫靶向治疗是目前治疗SNMM的重要策略之一,是改善患者预后的希望。Objective To investigate the prognosis and its influence factors of malignant melanoma of the nasal cavity and paranasal sinuses(SNMM).Methods A retrospective analysis method was used to analyze all the clinical data of 52 patients with SNMM.All the subjects of the study were patients who visited the First Affiliated Hospital of Zhengzhou University from October 2011 to October 2019.The Kaplan-Meier method and Cox regression model were used for univariate and multivariate survival analysis to obtain the factors affecting the prognosis of patients.Results Among patients with SNMM,there were 28 males and 24 females.The average age of the patients was 62.3 years,ranging from 6 to 82 years.The survival time was 5 to 96 months,the 5-year survival rate was 23.1%,and the median survival time was 29.3 months.Cox regression model analysis found that clinical staging,Ki-67 positive rate and adjuvant therapy were independent factors affecting the 5-year survival rate of patients with SNMM.Conclusion SNMM is highly invasive which has a high metastasis rate.It should be reviewed regularly after surgery and extended the follow-up time.Immunotargeted therapy is currently one of the important treatment strategies for SNMM,which is the hope of improving the prognosis of patients.
