睑裂斑对泪膜稳定性及眼表形态的影响

摘　　要：目的:检测并分析睑裂斑患者泪膜稳定性及眼表形态的改变。方法:横断面观察性研究2019年4月~6月期间在遂宁市第一人民医院确诊为睑裂斑的患者15例(20眼)作为睑裂斑组,正常人群16人(20眼)为对照组,分别对两组行泪液分泌试验(Schirmer′s test I,STI)、泪膜破裂时间(Tear break-up time,BUT)检查及角膜地形图检查,对比两组BUT、STI值及角膜地形图各项指标:角膜表面规则指数(Surface regularity index,SRI)、表面不规则指数(Surface asymmetry index,SAI)、模拟角膜表面散光值(Cylinder,CYL)、角膜预测视力(Potential visual acuity,PVA)。结果:睑裂斑组BUT、STI值均较对照组明显减少,角膜地形图SRI、SAI、CYL值均较对照组显著升高,而PVA却较对照组降低,差异均有统计学意义(P<0.05)。结论:角膜地形图检查可用于评价泪膜的稳定性及眼表形态规则性;睑裂斑所致的眼表结构、形态改变可导致眼表上皮完整性破坏及泪膜稳定性下降。临床上对睑裂斑患者进行干预治疗是有必要的。Objective Using corneal topography to evaluate alteration of tear stability and surface regularity in patients with pinguecula.Method Cross-sectional and observational study,15 patients(20 eyes)diagnosed with pinguecula at the department of ophthalmology,The first people′s hospital of Suining were enrolled as pinguecula group,and 16 healthy volunteers(20 eyes)as control group,Tear film break-up time examination(BUT),Schirmer′s testⅠ(STI)and the Surface regularity index(SRI)、Surface asymmetry index(SAI)、Cylinder(CYL)、Potential visual acuity(PVA)values of corneal topography were performed in both groups.Results BUT and STI values in pinguecula group were significantly smaller than those in control group(both P<0.05),the values of SRI、SAI and CYL in pinguecula group were significantly higher than those in control group,while the PVA value was significantly smaller than that in control group(P<0.05).Conclusion The corneal topography parameters were helpful for evaluation of tear film stability and surface regularity;Alteration of ocular surface structure and form caused by Pinguecula could lead to destruction of ocular surface integrity and tear film stability,therefore,clinic treatment of pinguecula is necessary.
