摘  要:目的利用静息态功能磁共振(rs-fMRI)探讨精神分裂症患者及高危人群脑区局部一致性(ReHo)。方法选取2010年3月至2019年10月期间就诊于中国医科大学附属第一医院精神医学科和沈阳市精神卫生中心的31例精神分裂症患者纳入患者组,选取23例患者组的直系健康家属(父母或子女)纳入高危人群组,同时招募115例志愿者纳入对照组。3组均进行脑区rs-fMRI检测,记录3组ReHo值并进行统计分析。结果与对照组比较,患者组中央前回的ReHo值显著下降(P<0.05)。与对照组比较,患者组和高危人群组右侧梭状回、右侧颞下回前部、右侧颞中回的ReHo值显著升高(均P<0.05);与高危人群组和对照组比较,患者组左侧枕上回、左侧枕中回、右侧枕中回、距状皮层、楔状叶区、右侧梭状回的ReHo值显著下降(均P<0.05)。结论精神分裂症患者及高危人群均存在脑功能异常。右侧梭状回、右侧颞下回前部、右侧颞中回可能是精神分裂症遗传素质性相关的脑区。ReHo值改变可能为精神分裂症早期预防及诊断提供参考。Objective Regional homogeneity(ReHo)was used to explore the brain function among individuals with schizophrenia,and those with a high genetic risk of developing schizophrenia.Methods The patients visiting the department of spirit of medicine,The First Hospital of China Medical University and Mental Health Center in Shenyang were enrolled into the study from March 2010 to October 2019.Thirty-one patients with schizophrenia comprised the SZ group,23 immediate family members(parents or children)of the patients were assigned to the high genetic risk of schizophrenia(GHR)group,at the same time 115 volunteers were recruited into the control group.Resting-state functional magnetic resonance imaging(rs-fMRI)was performed in all the 3 groups,and ReHo values were recorded and analyzed statistically.Results ReHo value of the anterior central gyrus in the SZ group was decreased compared to that in the control group(P<0.05).Compared with the control group,ReHo values in the SZ and GHR groups were significantly increased in the right fusiform,right anterior inferior temporal,and right middle temporal gyri.ReHo values of left superior occipital,left middle occipital,and right middle occipital gyri,talus cortex,cuneiform lobe area and right fusiform gyrus in the SZ group were significantly lower than those in the GHR and control groups(P<0.05).Conclusion rs-fMRI findings suggest that the patients with schizophrenia and GHR may have abnormal brain function.The right fusiform,the right anterior inferior temporal,and the right middle temporal gyri may be the potential susceptibility and image markers of the disease and evaluation of their ReHo values may help in early prevention and diagnosis of schizophrenia.
