摘  要:目的:对比免疫组织化学法(IHC)与荧光原位杂交技术(FISH)检测861例乳腺癌人类表皮生长因子受体2(HER-2)蛋白表达和基因状态的一致性。方法:参照《乳腺癌HER-2检测指南(2019版)》判读标准对乳腺癌石蜡组织采用IHC法及FISH法分别检测HER-2蛋白表达和HER-2基因状态,进行比较分析。结果:861例乳腺癌患者中,IHC HER-2(0/1+)和FISH(-)的一致率为97.1%(134/138);IHC HER-2(2+)和FISH(+)的一致率为28.7%(188/655);IHC HER-2(3+)和FISH(+)的一致率为98.5%(67/68)。两种检测方法的检测结果存在一致性,差异具有统计学意义(Kappa=0.137,P<0.0001)。结论:两种方法相结合,以便精准评价HER-2检测结果指导靶向治疗。Objective:To evaluate the concordance of HER-2/neu protein expression and gene status by immunohistochemistry(IHC)and fluorescence in situ hybridisation(FISH)in 861 cases of breast cancer patients.Methods:Referring to the guideline for HER-2 detection in breast cancer,the 2019 version,the protein expression and gene status of HER-2 in paraffin-embeded tissues of breast cancer were detected by IHC and FISH,respectively,and the concordance was evaluated.Results:Among 861 cases of breast cancer patients,the coincidence rate of IHC(0/1+)and FISH(-)was 97.1%(134/138).The coincidence rates of IHC(2+/3+)and FISH(+)were 28.7%(188/655)and 98.5%(67/68),respectively.The Kappa test result showed there was concordance between IHC and FISH(Kappa=0.137,P<0.0001).Conclusion:The combination of IHC and FISH should evaluate accurately the expression of HER-2 and guide targeting therapy.
