摘 要:目的:探讨气管镜下热冷联合消融治疗非小细胞肺癌(NSCLC)所致阻塞性肺不张的优势。方法:86例非小细胞肺癌(NSCLC)所致阻塞性肺不张患者,分为热消融组(43例)及热+冷消融组(43例),分组对比治疗前后的疗效、气促评分和并发症情况。结果:热消融治疗有效率为74.42%;热冷联合消融组治疗有效率为93.02%。热冷联合消融组及热消融组治疗疗效明显,可改善肺不张、降低气促评分、改善症状(P<0.05)。组间比较热冷联合消融组治疗有效率、肺复张率明显高于热消融组(P<0.05),在气胸、出血的并发症发生率上,热冷联合消融组与热消融组对比差异无统计学意义(P>0.05)。结论:气管镜下热冷联合消融较单独应用热消融能够使肿瘤快速、有效清除,对所致阻塞性肺不张能有效地复张。Objective:To investigate the advantages of combined thermal and cold ablation in the treatment of obstructive atelectasis caused by non-small cell lung cancer(NSCLC).Methods:86 patients with obstructive atelectasis caused by NSCLC were divided into thermal ablation group(43 patients)and combined thermal and cold ablation group(43 patients).Treatment effect,shortness of breath score and complications were compared between the two groups before and after treatment.Results:The effective rate was 74.42% in thermal ablation group and 93.02% in combined thermal and cold ablation group.The treatment effect of two groups was obvious,which can improve atelectasis,reduce shortness of breath score,and improve symptoms(P<0.05).The treatment efficiency and lung recruitment rate in the combined thermal and cold ablation group were significantly higher than those of the thermal ablation group(P<0.05).However,there was no significant difference in terms of the incidence of pneumothorax and bleeding complications(P>0.05).Conclusion:Tracheoscopy combined thermal and cold ablation can quickly and effectively clear the tumor compared with the application of thermal ablation alone,and recapitulate obstructive atelectasis.
