摘　　要：目的:探讨血液透析患者尿路上皮癌发生的相关因素、临床特点及治疗效果。方法:回顾性分析承德医学院附属医院泌尿外科2014年-2016年慢性肾功能衰竭维持性血液透析(MHD)合并尿路上皮癌患者12例,调查尿路上皮癌的发生情况、临床诊治特点及治疗效果。结果:血清C反应蛋白(CRP)、透析龄与尿路上皮癌发病呈正相关,透析时间是尿路上皮癌发病的独立危险因素。12例患者均接受手术治疗,其中除1例患者术后死亡外,其余患者均随访12个月,2例患者在术后12月内肿瘤复发,再次行经尿道膀胱肿瘤切除术,余9例患者均未见复发、转移。结论:透析时间越长越容易发生尿路上皮癌,提倡早期诊断、早期治疗。相对保守的外科手术治疗更合适于MHD患者,可大大降低手术风险。Objective:To investigate the factors,clinical features and therapeutic effects of urothelial carcinoma in hemodialysis patients.Methods:A retrospective analysis was performed on 12 patients with urothelial carcinoma who underwent maintenance hemodialysis(MHD)in patients with chronic renal failure from 2014 to 2016 in the department of urology,to investigate the occurrence,clinical diagnosis and treatment characteristics and therapeutic effects of urothelial carcinoma.Results:Serum c-reactive protein(CRP)and dialysis age were positively correlated with the incidence of urothelial carcinoma,and dialysis time was an independent risk factor for urothelial carcinoma.All the 12 patients received surgical treatment,among which 1 patient died after surgery,and the other patients were followed up for 12 months.2 patients had tumor recurrence within 12 months after surgery and underwent transurethral cystectomy again.No recurrence or metastasis was found in the remaining 9 patients.Conclusion:The longer dialysis time is,the easier it is to occur urothelial carcinoma.Early diagnosis and early treatment should be advocated.Relatively conservative surgical treatment is more suitable for MHD patients,which can greatly reduce the risk of surgery.
