摘 要:目的:探讨2型糖尿病(type 2 diabetes mellitus,T2DM)患者血糖控制前后血清肿瘤标志物CA125、CA19-9、CEA的动态变化,并分析其与平均血糖波动幅度(mean amplitude of glycemic excuisions,MAGE)的相关性。方法:入组2018年6月至2020年3月我院内分泌代谢病科初诊的T2DM患者106例,T2DM患者给予胰岛素治疗或口服降糖药物治疗或二者联合治疗,观察血糖控制前和控制后2周、1个月、3个月时CA125、CA19-9、CEA的动态变化,并分析血清肿瘤标志物与MAGE的相关性。结果:106例初诊T2DM患者中,男63例(59.43%),女43例(40.57%),男女比例为1.47∶1;患者年龄41~68岁,平均(50.81±11.19)岁。T2DM患者在入院时血糖控制前、血糖控制后2周、血糖控制后1个月、血糖控制后3个月的肿瘤标志物CA125、CA19-9和CEA水平逐渐降低,经重复测量方差分析均有统计学差异(均P<0.001)。血糖控制后2周、1个月、3个月时对T2DM患者行动态血糖监测评估MAGE,经Pearson相关性分析,MAGE水平与肿瘤标志物CA125、CA19-9和CEA水平均呈正相关关系(均P<0.001)。结论:部分T2DM患者的血清肿瘤标志物水平会出现升高,经降糖治疗控制血糖后2周,肿瘤标志物水平明显下降,如血糖控制后2周仍无明显下降甚至继续升高者需警惕合并恶性肿瘤风险。MAGE水平与肿瘤标志物CA125、CA19-9和CEA水平均呈正相关关系,MAGE异常对T2DM患者血清肿瘤标志物水平存在影响。Objective:To explore the dynamic changes of CA125,CA19-9,CEA in patients with type 2 diabetes before and after blood glucose control,and to analyze their correlation with mean amplitude of glycemic excuisions.Methods:106 T2DM patients were enrolled in the Department of Endocrinology and Metabolism from June 2018 to March 2020.T2DM patients were treated with insulin or oral hypoglycemic drugs or combination of the two.The changes of CA125,CA19-9 and CEA before and 2 weeks,1 month and 3 months after blood glucose control were observed,and the correlation between serum tumor markers and MAGE was analyzed.Results:Among 106 newly diagnosed T2DM patients,63(59.43%)were male,43(40.57%)were female,and the ratio of male to female was 1.47∶1.The patients were 41~68 years old,with an average age of(50.81±11.19)years.The levels of tumor markers CA125,CA19-9 and CEA in T2DM patients before admission,2 weeks after admission,1 month after blood glucose control and 3 months after blood glucose control gradually decreased,and there were statistical differences after repeated measurement analysis of variance(P<0.001).After 2 weeks,1 month and 3 months of blood glucose control,the dynamic blood glucose monitoring and evaluation MAGE were performed in T2DM patients.According to Pearson correlation analysis,MAGE level was positively correlated with tumor markers CA125,CA19-9 and CEA(P<0.001).Conclusion:The level of serum tumor markers will rise in some T2DM patients.The level of tumor markers will decrease significantly after 2 weeks of blood glucose control by hypoglycemic treatment.For example,if there is no significant decrease or even continues to increase after 2 weeks of blood glucose control,it is necessary to be aware of the risk of combined malignancy.The level of MAGE was positively correlated with the levels of CA125,CA19-9 and CEA.The abnormal MAGE had an effect on the level of serum tumor markers in T2DM patients.
