皮脂腺痣继发肿瘤21例临床病理分析

摘　　要：目的探讨皮脂腺痣继发肿瘤的临床表现与组织病理特征。方法回顾分析21例皮脂腺痣继发肿瘤患者的临床和病理资料。结果21例患者中,男12例,女9例,皮脂腺痣发生继发肿瘤的平均年龄为30.75岁。皮损位于头顶16例,位于面部5例。组织病理结果显示,皮脂腺痣继发肿瘤中,良性肿瘤为19例,常见肿瘤类型依次为毛母细胞瘤、乳头状汗管囊腺瘤、皮脂腺腺瘤。恶性肿瘤2例均为基底细胞癌。其中同一患者皮脂腺痣继发3种肿瘤者1例,继发2种肿瘤1例,继发1种肿瘤19例。所有患者手术扩大切除,随访均无复发。结论皮脂腺痣继发肿瘤发生年龄多在成人,常见部位为头顶部,本组患者最常见的继发肿瘤为毛母细胞瘤、乳头状汗管囊腺瘤和皮脂腺腺瘤。大多数为良性肿瘤,也可发生恶性肿瘤,值得临床重视。手术切除可治愈,预后良好。Objective To investigate the clinical manifestation and histopathological features of secondary neoplasms arising from nevus sebaceus.Methods The clinicopathological data of 21 patients with secondary neoplasms arising from nevus sebaceus were retrospectively analyzed.Results There were 21 patients,including 12 males and 9 females.The mean age of patients with secondary neoplasms arising from nevus sebaceus was 30.75 years.The lesions were located on the head in 16 cases and on the face in 5 cases.Histopathological results showed that 19 cases of secondary neoplasms arising from nevus sebaceus were benign.The most common types of the neoplasms were trichoblastoma,syringocystadenoma papilleferum and sebaceoma.Two cases of malignant neoplasms were both basal cell carcinoma.There were 3 types of neoplasms arising from nevus sebaceus in one case,2 types of neoplasms in one case,and 1 type of neoplasms in 19 cases.No recurrence was found in all patients.Conclusion Secondary neoplasms arising from nevus sebaceus mostly attack adults and the common site was the top of the head.The most common secondary tumors in this group are trichoblastoma,syringocystadenoma papilleferum and sebaceoma.Most of them are benign,but malignant neoplasms can also occur,which deserves special attention in clinical practice.Most of them can be cured by surgical resection with good prognosis.
