摘  要:目的:探讨遗忘型轻度认知功能障碍(amnestic mild cognitive impairment,aMCI)患者齿状核静息态功能连接改变及其与认知功能的相关性。方法:选取34例aMCI患者作为观察组,另选取34例同期健康老年人作为对照组。应用静息态功能磁共振成像(rs-fMRI)技术采集数据,比较两组双侧齿状核与全脑其他脑区功能连接差异;采用偏相关分析aMCI患者功能连接改变与临床量表评分及生物标志物之间的相关性。结果:与对照组相比,观察组左侧齿状核与左侧颞叶及右侧丘脑,右侧齿状核与左颞上回以及右侧小脑后叶连接增强(AlphaSim矫正,P<0.05,体素数>100)。简易精神状态评分(MMSE)及言语流畅性评分(VFT)与aMCI患者左侧齿状核和右侧丘脑之间的功能连接值呈正相关(r=0.399,P=0.026;r=0.383,P=0.025);右侧齿状核和左侧颞上回之间的功能连接值与听觉词语学习测验再认评分呈负相关(r=-0.552,P=0.001),与磷酸化tau蛋白呈正相关(r=0.357,P=0.045)。结论:aMCI患者出现特定小脑皮层功能连接增强现象,且与认知功能量表评分及生物标志物相关。Objective:To explore the changes of resting state functional connectivity of dentate nuc-leus in patients with amnestic mild cognitive impairment(aMCI),and to analyze its correlation with cognitive function.Methods:A total of 34 patients with aMCI were selected as the observation group,and another 34 healthy elderly people were selected as the control group.Data were acquired by resting-state functional magnetic resonance imaging(rs-fMRI)to compare the differences in functional connectivity between the bilateral dentate nuclei and other regions of the whole brain in the two groups;and partial correlation was used to analyze the correlation between functional connectivity and clinical scale scores and biomarker in patients with aMCI.Results:Compared with the control group,the connections between the left dentate nucleus and the left temporal lobe and the right thalamus,the right dentate nucleus and the left superior temporal gyrus and the right posterior cerebellar lobe were enhanced in the observation group(alphasim correction,P<0.05,voxel number>100).Mini-mental State Examination(MMSE)and verbal fluency test(VFT)were positively correlated with the functional connectivity between left dentate nucleus and right thalamus(r=0.399,P=0.026;r=0.383,P=0.025),while the functional connectivity between right dentate nucleus and left superior temporal gyrus was negatively correlated with auditory verbal learning for recognition(r=-0.552,P=0.001)and positively correlated with phosphorylated tau protein(r=0.357,P=0.045).Conclusion:The specific cerebellar-cortical functional connection was enhanced in the aMCI stage,and this enhancement is related to the cognitive function scale and biomarker.
