SMILE与TransPRK治疗中低度近视疗效对比

摘　　要：目的探讨飞秒激光微小切口角膜基质透镜取出术(SMILE)与经上皮准分子激光角膜切削术(TransPRK)治疗中低度近视的疗效。方法选择2018年7月—2019年5月在本院行激光治疗的47例(94眼)中低度近视患者作为研究对象,根据近视矫正术式不同分为SMILE组(SMILE治疗)和TransPRK组(TransPRK治疗)两组[SMILE组22例(44眼)、TransPRK组25例(50眼)]。比较两组患者术前及术后6个月的裸眼视力、等效球镜度(SE)、角膜像差的变化情况,并比较两组患者干眼参数。结果两组患者术后6个月裸眼视力及SE水平均较术前明显改善,差异具有统计学意义(P<0.05);但两组患者之间裸眼视力及SE水平无明显差异(P>0.05);术后两组患者的角膜彗差、球差及总高阶像差均较术前明显升高,差异具有统计学意义(P<0.05);两组患者术后角膜彗差无明显差异(P>0.05);但SMILE组患者术后角膜球差及总高阶像差均明显低于TransPRK组,差异具有统计学意义(P<0.05);术后两组患者的泪河高度无明显差异(P>0.05);但SMILE组患者的首次泪膜破裂时间与平均泪膜破裂时间均明显长于TransPRK组,差异具有统计学意义(P<0.05)。结论SMILE与TransPRK均能改善中低度近视患者视力水平和视觉质量,但SMILE对球差、高阶像差的影响较TransPRK更小,术后干眼症状更轻。Objective To investigate the effect of femtosecond laser small incision keratomileusis(SMILE)and TransPRK in the treatment of moderate and low myopia.Methods 47 patients(94 eyes)with middle or low myopia those who were treated by laser in our hospital from July 2018 to may 2019 were enrolled in the study.According to the surgical method they were divided into SMILE group(treated with SMILE)and TransPRK group(treated with TransPRK),there were 22 patients in SMILE group(44 eyes)and 25 patients in TransPRK group(50 eyes).The changes of naked eye vision,spherical equivalent degree(SE)and corneal aberration of the two groups were compared before operationand 6 months after operation.The dry eye parameters were compared between the two groups.Results The visual acuity and SE level of the two groups were significantly improved 6 months after operation(P<0.05),but there was no significant difference between the two groups(P>0.05).The corneal coma,spherical aberration and total higher-order aberrations in the two groups were significantly higher than those before operation(P<0.05),there was no significant difference in corneal coma between the two groups after surgery(P>0.05),but the spherical aberration and total higher-order aberrations in the SMILE group were significantly lower than those in the TransPRK group(P<0.05);the lacrimal river height in the latter two groups was not significantly different(P>0.05),but the time the first tear film rupture and the mean time of tear rupture in SMILE groupwere significantly longer those in TransPRK group(P<0.05).Conclusions Both SMILE and TransPRK could improve the vision level and visual quality of the patients with middle to low myopia,but the influence of SMILE on spherical aberration and higher-order aberrations is smaller than TransPRK,achieved lighterdegree of postoperative dry eye.
