河北省冠心病患者远期抑郁现状及其影响因素分析

摘　　要：目的了解河北省冠心病患者出院1年后远期抑郁现状及影响因素,为实施有效心理干预提供依据。方法采用问卷调查法对河北省出院1年后冠心病进行抑郁症状调查和收集一般资料,描述抑郁现状并分析其影响因素。结果河北省冠心病患者远期抑郁人数为650例(55.0%)。多因素分析结果显示,性别(OR=0.718,95%CI:0.545~0.946)、县区居住(OR=0.713,95%CI:0.537~0.947)、支架/搭桥治疗(支架:OR=3.328,95%CI:1.215~9.115;搭桥:OR=4.635,95%CI:1.710~12.562)、饮酒(OR=1.472,95%CI:1.046~2.071)、中/低等体力活动(中等:OR=3.097,95%CI:1.451~6.610;低等:OR=1.472,95%CI:1.041~2.083)、心脏康复知晓情况(部分知晓:OR=0.586,95%CI:0.386~0.891;不知晓:OR=1.377,95%CI:1.018~1.863)是影响因素。结论河北省冠心病患者远期抑郁发生率较高,性别、居住地、治疗、饮酒、体力活动、心脏康复知晓情况为主要影响因素,应予关注。Objective To understand the current status and influencing factors of long-term depression in patients with coronary heart disease in Hebei Province in order to provide basis for effective mental intervention.Methods A questionnaire survey method was used to investigate depression symptoms and collect general data of coronary heart disease one year after discharge from the hospital in three regions of Hebei Province,describe the current status of depression,and analyze its influence factors.Results A total of 650 patients with coronary heart disease had long-term depression in Hebei Province(55.0%).Multivariate logistic regression analysis showed that gender(OR=0.718,95%CI:0.545-0.946),county residence(OR=0.713,95%CI:0.537-0.947),stent/cardiac bypass therapy(stent:OR=3.328,95%CI:1.215-9.115;cardiac bypass:OR=4.635,95%CI:1.710-12.562),drinking(OR=1.472,95%CI:1.046-2.071),physical activity(moderate intensity:OR=3.097,95%CI:1.451-6.610;low intensity:OR=1.472,95%CI:1.041-2.083),and awareness of cardiac rehabilitation(partial:OR=0.586,95%CI:0.386-0.891;unknown:OR=1.377,95%CI:1.018-1.863)were influence factors.Conclusion The incidence of long-term depression is high in patients with coronary heart disease in Hebei Province.Gender,residence,treatment,drinking,physical activity and knowledge of cardiac rehabilitation are main influence factors and should be given attention.
