动脉瘤性蛛网膜下腔出血致急性脑积水的危险因素分析

摘　　要：目的分析动脉瘤性蛛网膜下腔出血(aSAH)致急性脑积水的危险因素。方法选取2019-06—2020-06郑州大学第一附属医院收治的动脉瘤性蛛网膜下腔出血致急性脑积水的患者40例为观察组,另选取同时间段未发生急性脑积水的患者40例为对照组,采用单因素相关分析和Logistic回归分析对2组患者的相关变量进行回顾性分析。结果高血压史、后循环动脉瘤、脑室内积血、高Hunt-Hess分级的单因素分析差异有统计学意义(P<0.05)。高血压史、后循环动脉瘤、脑室内积血、高Hunt-Hess分级等多因素Logistic回归分析OR值分别为19.816、7.218、34.433、8.232,差异有统计学意义(P<0.05)。结论高血压史、后循环动脉瘤、脑室内积血、Hunt-Hess分级是aSAH后急性脑积水形成的危险因素,应对有危险因素的患者及早行腰大池引流术或脑室外引流以减少急性脑积水的发生风险。Objective To analyze the risk factors for acute hydrocephalus caused by aneurysmal subarachnoid hemorrhage(aSAH).Methods Observation group were 40 cases of acute hydrocephalus due to aSAH from June 2019 to June 2020 in our hospital.Randomly select another 40 cases during the same period did not form acute hydrocephalus as control group.Retrospectively analyze related factors by univariate analysis and Logistic regression analysis.Results The P values of univariate analysis for hypertension,posterior circulation aneurysms,cisternal hemorrhage,high grade of Hunt-Hess were all less than 0.05 with statistical significance.The OR values of Logistic regression analysis for hypertension,posterior circulation aneurysms,cisternal hemorrhage,high Hunt-Hess grades were 19.816,7.218,34.433,8.232 with statistical significance(P values were all less than 0.05).Conclusion hypertension,posterior circulation aneurysms,cisternal hemorrhage and high Hunt-Hess grades are risk factors of acute hydrocephalus caused by aSAH.Clinicians should take measures such as lumbar drainage or extraventricular drainage to reduce the chance for acute hydrocephalus.
