Aptima、HC2检测对CIN2及以上宫颈病变的诊断效果

摘　　要：目的:探讨HPV E6/E7 mRNA(Aptima)和HPV DNA二代杂交捕获(HC2)检测对宫颈上皮内瘤变2级(CIN2)及以上病变的诊断价值。方法:选取2018年1月—2019年6月在本院就诊的疑似宫颈病变患者100例,给予Aptima和HC2检测并与病理结果比较。结果:最终确诊慢性炎症9例,CIN1级23例,CIN2级34例,CIN3级22例,宫颈癌12例;CIN2、CIN3和宫颈癌患者HPV E6/E7 mRNA分别为[9003.82(6780.88,11021.80)copies/ml,27801.19(16781.29,32011.88)copies/ml和6770.28(4100.28,7220.22)copies/ml],高于慢性炎症、CIN1患者(P<0.05);Aptima检测诊断CIN2及以上病变的灵敏性(82.4%)低于HC2检测(94.1%),而特异性(56.3%)高于HC2检测(21.9%)(P<0.05),诊断CIN2及以上病变的阳性预测值和阴性预测值两种检测无差异(P>0.05)。结论:Aptima与HC2检测在CIN2及以上宫颈病变中有一定诊断效果。Objective:To explore the value of detection of HPV E6/E7 mRNA(Aptima)and HPV DNA second generation hybridization capture(HC2)for diagnosing lesions with≥cervical intraepithelial neoplasia(CIN)Ⅱgrade.Methods:100 patients with suspected cervical lesions were selected and were given Aptima and HC2 detections,and which’s results were compared with pathological results from January 2018 to June 2019.Results:Among the 100 patients,there were 9 patients with chronic inflammation,23 patients with CINⅠ,34 cases of patients with CINⅡ,22 patients with CINⅢ,and 12 patients with cervical cancer.The HPV E6/E7 mRNA of the patients with CINⅡ,CINⅢ,and cervical cancer were 5102.39(3378.82,7800.83)copies/ml,27801.19(16781.29,32011.88)copies/ml,and 6770.28(4100.28,7220.22)copies/ml,respectively,which were significant higher than those of the patients with chronic inflammation and CINⅠ(P<0.05).The sensitivity of Aptima for diagnosing cervical CINⅡ,CINⅢ,and cervical cancer was 82.4%,which was significant lower than that(94.1%)of HC2(P<0.05),while there were no significant differences in the positive predictive value and negative predictive value between of Aptima and HC2 for diagnosing cervical CINⅡ,CINⅢ,and cervical cancer(P>0.05).Conclusion:The detection of Aptima and HC2 has some effect for diagnosing CINⅡ,CINⅢ,and cervical cancer.
