摘 要:目的:探讨经导管动脉内栓塞(TAE)联合小干扰RNA沉默基因治疗肝癌的疗效及对兔缺氧诱导因子-1(HIF-1α)、血管内皮生长因子(VEGF)的影响。方法:以20只肝癌模型兔为研究对象,采用随机数字表法分为两组,每组10只,对照组采用动脉内注入PVA微粒与对比剂混悬液治疗,研究组使用动脉内灌注PVA微粒与对比剂混悬液及HIF-1α-shRNA溶液治疗,观察对肿瘤体积、肝癌组织核分裂像总数、HIF-1α蛋白和VEGF蛋白的表达以及门冬氨酸氨基转移酶(AST)和丙氨酸基转移酶(ALT)水平的影响。结果:(1)治疗前两组肿瘤体积分别为(5510.2±1302.8)mm^(3)和(5475.3±1237.7)mm^(3),两组比较无统计学差异(P>0.05);治疗后两组肿瘤体积均有一定程度的增加,与治疗前比较,对照组为(8125.1±1114.3)mm^(3),差异有统计学意义(P<0.05),研究组为(6314.4±1337.6)mm^(3),差异无统计学意义(P>0.05),组间比较研究组术后肿瘤体积显著降低(P<0.05)。(2)对照组核分裂像总数为(11.12±3.28)个,研究组为(7.85±2.17)个,研究组核分裂像总数显著低于对照组(P<0.05)。(3)研究组肿瘤组织HIF-1α表达量为(1.73±0.15)pg/ml,显著低于对照组的(7.15±2.38)pg/ml(P<0.05),研究组肿瘤组织VEGF表达量为(1.51±0.64)pg/ml,显著低于对照组的(8.75±1.36)pg/ml(P<0.05)。(4)治疗前两组AST和ALT水平比较均无统计学差异(P>0.05),治疗后对照组和研究组ALT水平分别为(78.51±4.85)U/L和(43.58±4.14)U/L,均显著升高(均P<0.05),对照组和研究组AST水平分别为(71.35±4.15)U/L和(38.27±3.24)U/L,均显著升高(均P<0.05),与对照组相比,研究组术后ALT和AST水平均显著降低(均P<0.05)。结论:TAE联合siRNA干扰HIF-1α能通过降低HIF-1α和VEGF蛋白表达量抑制肝癌兔肿瘤的生长。Objective:To explore the curative effect of transcatheter arterial embolization(TAE)combined with small interfering RNA(siRNA)in the treatment of hepatocarcinoma rabbits and its influence on hypoxia-inducible factor-1(HIF-1α)and vascular endothelial growth factor(VEGF).Methods:Taking 20 liver cancer model rabbits as the research objects,and they were divided into two groups by random number table method,with 10 rabbits in each group.The control group was treated with intra-arterial injection of PVA particles and contrast medium suspension,while the study group was treated with intra-arterial infusion of PVA particles,contrast medium suspension and HIF-1α-shRNA solution.The effects on tumor volume,total number of mitotic figures in liver cancer tissue,the expression of HIF-1αprotein and VEGF protein,and the levels of aspartate aminotransferase(AST)and alanine transferase(ALT)were observed.Results:The tumor volumes of the two groups before treatment were(5510.2±1302.8)mm 3 and(5475.3±1237.7)mm 3 respectively,and there was no statistical difference between the two groups(P>0.05).The tumor volume of the two groups increased to a certain extent after treatment,and the control group was(8125.1±1114.3)mm 3,which was statistically different from that before treatment(P<0.05);the study group was(6314.4±1337.6)mm 3,which was not statistically different from that before treatment(P>0.05);compared with the control group,the tumor volume in the study group was significantly reduced after surgery(P<0.05).The total number of mitotic images in the control group was(11.12±3.28),and the total number of mitotic images in the study group was(7.85±2.17),and the total number of mitotic images in the study group was significantly lower than that in the control group(P<0.05).The expression of HIF-1αin tumor tissues of the study group was(1.73±0.15)pg/ml,which was significantly lower than that of the control group[(7.15±2.38)pg/ml](P<0.05).The expression of VEGF in tumor tissues of the study group was(1.51±0.64)pg/ml,which w
