摘 要:目的探讨循环肿瘤细胞(CTCs)、热休克蛋白(HSP)32、HSP27在食管癌(EC)同步放化疗中动态变化及临床意义。方法选取本院收治的107例EC同步放化疗患者,根据同步放化疗后肿瘤组织病理反应分为完全缓解组(n=42)、非完全缓解组(n=63)。比较两组外周血CTCs、HSP32、HSP27变化并分析临床意义。结果随化疗进行,CTCs、HSP32、HSP27呈降低趋势,完全缓解组低于非完全缓解组,差异均有统计学意义(P<0.05);化疗1、2个疗程后CTCs、HSP32、HSP27均是非完全缓解的相关因素,且化疗2个疗程后数据预测价值更高(P<0.05);CTCs、HSP32、HSP27高危患者生存率低于低危患者,差异有统计学意义(P<0.05)。结论 EC患者同步放化疗中外周血CTCs、HSP32、HSP27动态变化可预测同步放化疗疗效及预后转归,对指导临床个体化完善治疗方案具有重要意义。Objective To investigate the dynamic changes and clinical significance of circulating tumor cells(CTCs),heat shock protein(HSP)32,and HSP27 in concurrent radiotherapy and chemotherapy of esophageal cancer(EC).Methods A total of 107 EC patients with concurrent radiotherapy and chemotherapy were selected.According to the pathological response of tumor tissue after concurrent radiotherapy and chemotherapy,they were divided into the complete remission group(n=42)and the incomplete remission group(n=63).The changes in peripheral blood CTCs,HSP32,and HSP27 of the two groups before and after treatment were compared and the clinical significance was analyzed.Results With the progress of chemotherapy,CTCs,HSP32,and HSP27 showed a decreasing trend,and the complete remission group was lower than that of the incomplete remission group,and the difference was statistically significant(P<0.05).CTCs,HSP32 and HSP27 after 1 and 2 courses of chemotherapy were all related factors for incomplete remission,and the predictive value of the data after 2 courses of chemotherapy was higher(P<0.05).The survival rate of CTCs,HSP32,HSP27 high-risk patients was lower than that of low-risk patients,and the difference was statistically significant(P<0.05).Conclusion The dynamic changes of peripheral blood CTCs,HSP32 and HSP27 during concurrent radiotherapy and chemotherapy of EC patients can predict the curative effect and prognosis of concurrent radiotherapy and chemotherapy,which is of great significance for guiding the clinical individualized treatment plan.
