摘 要:目的通过建立大鼠心肌缺血再灌注损伤(I/RI)的模型,探讨槲寄生联合远程缺血后适应(RIPOC)对大鼠心肌是否具有协同保护作用。方法50只SD大鼠随机分5组,每组10只:假手术组(Sham组)、缺血再灌注损伤组(I/RI组)、槲寄生组、远程缺血后适应组(RIPOC组)、联合组(槲寄生+RIPOC组)。记录开胸前基础值(T0),缺血10 min(T1),30 min(T2),和再灌后30 min(T3),60 min(T4),120 min(T5)的心率(HR)、平均动脉压(MAP)。再灌结束后检测各组大鼠血清肌酸激酶(CK)、肌酸激酶心肌型同工酶(CK-MB)、乳酸脱氧酶(LDH)的含量的变化,并测定心肌缺血、梗死程度。结果HR:T1至T5时刻,与I/RI组比较,各干预组(槲寄生组、RIPOC组和联合组,下同)HR相对较高(P<0.05,P<0.01)。MAP:T2至T5时刻,与I/RI组比较,各干预组大鼠MAP相对较高(P<0.01)。心肌酶指标:与I/RI组比较,各干预组大鼠的三种血清酶浓度均有所降低(P<0.01)。心肌梗死程度:与I/RI组比较,各干预组大鼠的梗死程度均明显降低(P<0.01)。以上结果中,联合组大鼠的效果最好。结论槲寄生和RIPOC联合应用可以改善I/RI大鼠血流动力学,还可以减少心肌酶的释放,改善再灌注后心肌损伤,对I/RI的心肌有一定的保护作用。Objective To investigate whether mistletoe combined with remote ischemic post-conditioning(RIPOC)has synergistic protective effects on rat myocardium by establishing an ischemia-reperfusion injury(I/RI)model.Methods A total of 50 SD rats were randomly divided into the sham operation group(the sham group,n=10),the ischemia-reperfusion injury group(the I/RI group,n=10),the mistletoe group(n=10),the remote ischemic postconditioning group(the RIPOC group,n=10)and the combined group(the mistletoe combined with RIPOC group,n=10).Heart rate(HR)and mean arterial pressure(MAP)were recorded before thoracotomy(T0),10 min(T1)and 30 min(T2)after ischemia,and 30 min(T3),60 min(T4)and 120 min(T5)after reperfusion.At the end of reperfusion,the changes of serum creatine kinase(CK),creatine kinase isoenzyme MB(CK-MB)and Lactic dehydrogenase(LDH)were detected,and the degree of myocardial ischemia and infarction was measured.Results For HR,from T1 to T5,compared with the I/RI group,the HR of each intervention group(the mistletoe group,the RIPOC group and the combined group,similarly hereinafter)was relatively higher(P<0.05,P<0.01).For MAP,from T2 to T5,compared with the I/RI group,the MAP of rats in each intervention group was relatively higher(P<0.01).For myocardial enzymes indices,compared with the I/RI group,the concentration of three serum enzymes in each intervention group of rats all decreased to some extent(P<0.01).For the degree of myocardial infarction,compared with the I/RI group,the degree of myocardial infarction in each intervention group of rats all decreased significantly(P<0.01).Among the above results,the combined group of rats has the best effect.Conclusion The combined application of mistletoe and RIPOC can improve the hemodynamics of rats in the I/RI group,reduce the release of myocardial enzymes,improve myocardial injury after reperfusion,and protect the my ocardium of rats in the I/RI group to some extent.
