摘　　要：目的探讨关节镜下克氏针张力带钢丝固定治疗青少年骨骺未闭型胫骨髁间嵴骨折的应用效果。方法选取2013年1月至2019年4月收治的骨骺未闭的青少年胫骨髁间嵴骨折患者46例,按照盲选随机的原则以抽签法分成两组,其中对照组(n=23)以高强线“8”字缠绕固定治疗,观察组(n=23)在关节镜下克氏针张力带钢丝固定治疗,观察不同固定方式对骨折复位优良率、膝关节功能影响、自主生活能力及中远期生活质量的影响。结果观察组、对照组固定复位优良率为95.65%、73.91%,两组差异有统计学意义(P<0.05)。观察组膝关节功能HSS评分高于对照组,差异有统计学意义(P<0.05)。观察组患者自主生活ADL量表评分总分高于对照组,差异有统计学意义(P<0.05)。观察组患者生活质量SF-36量表评分高于对照组,差异有统计学意义(P<0.05)。观察组患者术后并发症总发生率低于对照组,但差异无统计学意义(P>0.05)。结论在关节镜下克氏针张力带钢丝固定治疗青少年骨骺未闭型胫骨髁间嵴骨折,不仅避免损伤骨骺,同时复位固定效果理想。患者术后关节功能恢复好,术后并发症风险小,为患者生活质量、自主生活能力的提升打下坚实基础,值得推广应用。Objective To investigate the application efficacy of arthroscopic Kirschner wire tension band steel wire fixation in the treatment of juvenile patent epiphysis type tibial intercondylar crest fracture.Methods A total of 46 cases of juvenile patent epiphysis type tibial intercondylar crest fracture admitted from January 2013 to April 2019 were selected and divided into the control group(n=23)and the observation group(n=23)according to the principle of blind random selection by drawing lots.Among them,the control group was treated with high-strength wire"8"wound fixation,while the observation group was treated with arthroscopic Kirschner wire tension band steel wire fixation.The impacts of different fixation methods on the excellent and good rate of fracture reduction,the impacts of knee joint function,the ability of independent living and the medium-and long-term quality of life were observed.Results The excellent and good rates of fixed reduction in the observat ion group and the control g roup were 95.65%and 73.91%,respectively,with statistically significant differences between the two groups(P<0.05).The HSS score of knee joint function in the observation group was higher than those in the control group,with statistically significant differences(P<0.05).The total score of activity of daily living(ADL)scale of patients in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).The scores of SF-36 scale of quality of life in the observati on group were higher than those in the control group in the comparisons,such as aspect from physiological functions,with statistically significant differences(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,without statistically significant difference(P>0.05).Conclusion Arthroscopic Kirschner wire tension band steel wire fixation can not only avoid injury to epiphysis,but also achieve ideal reduction and fixation efficacies.The joint function of p
