摘 要:子宫内膜癌是一类异质性显著的常见恶性肿瘤,识别其分子生物学特征能更有针对性地治疗患者,改善患者预后。该文综述子宫内膜癌最经典的3种分类系统:Bokhman二分法、癌症基因组图谱分子分类法,以及子宫内膜癌前瞻性分子危险分类法(Proactive Molecular Risk Classifier for Endometrial Cancer,ProMisE)。其中,ProMisE分类系统将子宫内膜癌分成4个亚组:错配修复缺失亚组、DNA聚合酶-?核酸外切酶突变亚组、p53突变型亚组和p53野生型亚组。ProMisE分子分类法适用于小活检和石蜡包埋组织,我国三甲医院能实施这种分子分类,该分类法的广泛应用将极大提高我国子宫内膜癌的精准诊治水平。Endometrial cancers,a group of conspicuous heterogeneous malignant tumors,occurred commonly in women.How to distinguish the molecular features and personalize the patients with suitable treatment is an urgent mission for our pathologists and oncologists nowadays.We reviewed the published articles associated with endometrial cancers and molecular classification,and analyzed the progress and advantages of different classifications.There are three classic different classifications:Bokhman’s binary classifications,The Cancer Genome Atlas(TCGA)-based molecular classifications and Proactive Molecular Risk Classifier for Endometrial Cancer(ProMisE).ProMisE includes four subgroups:mismatch repair-deficient,polymerase-Epsilon exonuclease domain mutations,p53 mutations and p53 wild type.ProMisE is a practical classification system which uses the formalin-fixed and paraffin embedding tissues from biopsy.The ProMisE system can be carried out at our pathological laboratory of Tertiary Grade A Hospitals,which will improve the precision medicine for diagnosis and treatment.
