血糖、胆固醇水平与肝衰竭患者预后的相关性分析

摘　　要：目的探讨血糖、胆固醇水平与肝衰竭患者预后的相关性。方法收集2018年1月至2019年12月于辉县市人民医院接受治疗后预后不良的78例肝衰竭患者的临床资料,作为预后不良组;收集同期于我院接受治疗后预后良好的122例肝衰竭患者的临床资料,作为预后良好组。记录两组空腹血糖、总胆固醇水平,分析肝衰竭患者空腹血糖、总胆固醇水平与其预后的相关性。结果预后不良组空腹血糖水平[(3.49±0.86)mmol/L]、总胆固醇水平[(1.05±0.49)mmol/L]低于预后良好组[(4.82±1.35)mmol/L、(2.26±0.54)mmol/L],差异有统计学意义(P<0.05);将空腹血糖、总胆固醇水平分别作为自变量,将预后结局作为因变量,将预后良好赋值为1,预后不良赋值为2,得到Logistics回归方程:Y=9.178-2.245X1-1.742X2。空腹血糖低水平、总胆固醇低水平是导致肝衰竭患者预后不良的危险因素(OR>1,P<0.05)。结论血糖及总胆固醇是反映肝衰竭患者肝功能损伤严重程度、病情进展、预后评估的重要指标,通过对上述两项指标可对患者疾病严重程度及预后做出准确判断。Objective To investigate the correlation of blood glucose and cholesterol levels with the prognosis of patients with liver failure.Methods The clinical data of 78 patients with liver failure who received treatment in the People’s Hospital of Huixian from January 2018 to December 2019 were collected as the poor prognosis group;122 patients with good prognosis who received treatment in the People’s Hospital of Huixian during the same period were collected as the good prognosis group.The fasting blood glucose and total cholesterol levels of the two groups were recorded,and the correlation of the fasting blood glucose and total cholesterol levels with the prognosis of patients with liver failure was analyzed.Results The fasting blood glucose level[(3.49±0.86)mmol/L]and total cholesterol level[(1.05±0.49)mmol/L]of the poor prognosis group were lower than those of the good prognosis group[(4.82±1.35)mmol/L,(2.26±0.54)mmol/L],the differences were significant(P<0.05);the fasting blood glucose and total cholesterol levels were used as independent variables,and the prognostic outcome was used as the dependent variable.The good prognosis was assigned a value of 1,and the poor prognosis was assigned a value of 2,and the logistic regression equation was obtained:Y=9.178-2.245 X1-1.742 X2.The results showed that low level of fasting blood glucose and low level of total cholesterol were the risk factors for poor prognosis of patients with liver failure(OR>1,P<0.05).Conclusions Blood glucose and total cholesterol are important indicators that reflect the severity of liver damage,disease progression,and prognosis evaluation in patients with liver failure.The detection of the above two indicators can make accurate judgments on the severity and prognosis of patients.
