摘 要:目的:评估实体瘤患儿营养风险,进行营养评价及血清维生素状况分析。方法:收集2017年7月—2018年6月间北京儿童医院肿瘤外科收治的205例实体肿瘤患者,进行STAMP营养不良风险筛查及营养评估(体格指标、血清蛋白、前白蛋白),完善血清9种维生素(维生素A、维生素D、维生素E、维生素C、维生素B 1、维生素B 2、维生素B 6、维生素B 9、维生素B 12)浓度测定及分析。进一步分析不同年龄段儿童营养状况及不同营养状况与住院时长的相关性。结果:205例实体瘤患儿中,男童97例(47.3%)、女童108例(52.7%),年龄范围4月龄~16岁9月龄。中度营养不良风险者148例(72.2%)、高度营养不良风险者57例(27.8%),营养不良风险与年龄(r=0.230,P=0.001)及住院时长呈正相关(r=0.144,P=0.04)。低白蛋白血症者31例(15.1%),住院时长显著增加,低血清前白蛋白者150例(73.2%)。维生素检测显示:维生素D缺乏最广泛,占比83.4%,其次为维生素C(15.6%)、维生素A(9.3%)、维生素B 6(9.3%)、维生素B 2(5.4%),其余维生素的缺乏率在5%以下。结论:实体肿瘤患儿存在较高营养不良风险,还存在不同程度的低白蛋白血症、低前白蛋白、血清维生素缺乏等。营养状况与住院时长存在相关性,需进行早期营养不良风险筛查及评估,以及早期营养干预,改善患者临床结局。Objective To investigate the nutritional risk,nutritional status and vitamin concentrations in blood of children with solid tumors.Method A total of 205 children with solid tumors at the department of surgical oncology in Beijing Children's Hospital from July 2017 to June 2018 were selected.Nutritional risk screening tool was STAMP and evaluated the nutritional status with physical parameters,serum albumin and pre-albumin.Serum concentrations of 9 kinds of vitamins including vitamin A,D,E,C,B 1,B 2,B 6,B 9,B 12 were measured and the vitamin nutritional status were analyzed,then we further analyzed the nutritional status in children of various age groups,and explored the relationship between different status and the hospitalization time.Result Among the 205 children aged from 4 months old to 16 years and 9 months old,97 cases were boy,accounting for 47.3%,108 cases were girl,accounting for 52.7%.Among them,totally 148 patients were moderate nutritional risk(72.7%),57 children were severe nutritional risk(27.8%).Malnutritional risk was positively correlated with age(r=0.230,P=0.001)and the hospitalization time(r=0.144,P=0.04).Totally 31 patients(15.1%)were hypoalbuminema,the hospitalization time increased correspondingly.Totally 150 children(73.2%)were hypo-pre-albuminemia.The result of serum vitamin test showed that the vitamin D was the most deficiency one(83.4%),then the vitamin C(15.6%),vitamin A(9.3%),vitamin B 6(9.3%),vitamin B 2(5.4%),and the rest were all under 5%.Conclusion Solid tumor children have high incidence of malnutritional risk,meanwhile,varying degrees of hypoalbuminema,hypo-pre-albuminemia and vitamin deficiency.There was correlation between malnutritional status and the hospitalization time.We should screen malnutritional risk in early stage and support nutritional treatment in time,as to improve the patient's outcome.
