摘　　要：中医脾藏象理论是一个十分完整的体系,但在传承的过程中多数医家对“脾虚”概念进行了充分的阐述发挥,鲜少提及“脾实”,而近来多项研究表明“脾实”与多种代谢性疾病的发病相关。本文即通过梳理相关文献,系统探讨了“脾实”和“泻脾”的内涵以及相应方药。“脾实”即指生理性脾气充盛或病理性实邪壅脾,后者与代谢性疾病的发生密切相关;“泻脾”之法可分为祛脾湿、泻脾火和泻脾气三大类,泻脾之方药除前述三类外尚有专属“泻脾”“攻脾”之品。明确“脾实”与“泻脾”的内涵,有助于丰富脾藏象理论及相关疾病的辨证论治,对于中医临床具有重要现实意义。The theory of spleen viscera manifestation is a very complete system,in the process of inheritance,most physicians fully elaborated on the concept of"spleen deficiency"and rarely mentioned"spleen excess",however,many recent studies have shown that the latter is related to various metabolic diseases.This article discussed the connotation of"spleen excess"and"purging spleen"and corresponding Chinese Medicine through combing related literature."Spleen excess"refers to physiological fullness or pathological congestion,the latter is related to the metabolic diseases closely.The method of"purging spleen"can be divided into three categories:dispelling dampness、purging fire and moving Qi,the corresponding to medicine.Clarify the connotation of"spleen excess"and"purging spleen",it is helpful to enrich the theory of spleen viscera manifestation and the treatment of related diseases,and has important practical significance for clinical.
