医护患一体化护理在子宫腺肌病患者高强度聚焦超声治疗围术期疼痛管理中的应用

摘　　要：目的探讨医护患一体化护理模式在高强度超声聚焦治疗子宫腺肌病患者的围术期疼痛管理中的应用效果。方法选取首都医科大学附属北京妇产医院妇科微创中心2016年5月—2019年5月收治的子宫腺肌病并接受高强度聚焦超声(HIFU)治疗的患者102例,通过计算机随机分为研究组和对照组,并实施分组护理。对照组51例应用常规护理模式,研究组51例采用医护患一体化护理模式对围术期疼痛进行联合管理。比较两组患者术后疼痛评分、镇痛药物使用情况、焦虑度以及护理满意度护理效果。结果两组术后2 h视觉模拟评分(VAS)比较,差异无统计学意义(P>0.05)。两组术后24 h VAS均显著性低于术后2 h,且研究组低于对照组,差异有统计学意义(P<0.05或P<0.01)。研究组焦虑评分低于对照组,研究组术后使用镇痛药物患者比例低于对照组,差异有统计学意义(P<0.05)。研究组护理满意度高于对照组,差异有高度统计学意义(P<0.01)。结论对接受高强度聚焦超声治疗的子宫腺肌病患者实施医护患一体化疼痛管理模式,可以有效帮助患者正确解读及评价治疗中所产生的疼痛,从而提高护理疼痛管理水平,缓解患者术后疼痛、焦虑,减少药物使用以及提高患者满意度。Objective To explore the application and effect of integrated medical care model in the perioperative pain management of patients with adenomyosis treated by high-intensity focused ultrasound.Methods A total of 102 patients with adenomyosis who received high-intensity focused ultrasound(HIFU)treatment from the Minimally Invasive Center of Gynecology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University were selected from May 2016 to May 2019.They were randomly divided into study group and control group by computer,and implemented group nursing.The 51 cases in the control group were used the conventional nursing model,and the 51 cases in the study group were used the integrated medical care model for perioperative pain management.The postoperative pain scores,the use of analgesics,anxiety,and nursing satisfaction were compared between the two groups.Results There was no significant difference in visual analogue scale(VAS)between the two groups at 2 h after surgery(P>0.05).VAS at 24 h after surgery in both groups were significantly lower than those at 2 h after surgery,while the study group was lower than that in the control group,and the differences were statistically significant(P<0.05 or P<0.01).The anxiety score of the study group was lower than that of the control group,while the proportion of patients in the study group who used analgesics after surgery was lower than that of the control group,and the differences were statistically significant(P<0.05).The nursing satisfaction of the study group was higher than that of the control group,and the difference was highly statistically significant(P<0.01).Conclusion For patients with adenomyosis receiving high-intensity focused ultrasound treatment,the doctor-nurse-patient integrated mode could effectively help patients correctly interpret and evaluate the pain generated during treatment,thereby improving nursing pain management,alleviating postoperative pain and anxiety,reducing drug use and improving patient satisfaction.
