顺铂腹腔热灌注化疗联合深部热疗治疗腹膜假粘液瘤的护理分析

摘　　要：目的探讨腹膜假粘液瘤患者行腹腔热灌注化疗联合深部热疗的近期疗效,总结热化疗的护理要点和体会.方法回顾性分析47例行顺铂(35~40 mg/m^(2))腹腔热灌注化疗联合深部热疗的腹膜假粘液瘤患者的临床资料,治疗过程中采取针对性护理措施,统计近期疗效并观察不良反应.结果47例患者均顺利完成顺铂腹腔热灌注化疗联合深部热疗>2个疗程,其中完全缓解(CR)6例,部分缓解(PR)28例,稳定(SD)8例,有效率为72.3%,临床获益率(CBR)为89.4%.21例患者治疗后隔天出现Ⅰ~Ⅱ度胃肠道反应,表现为轻度恶心呕吐、食欲下降;5例患者治疗后3周出现骨髓抑制(白细胞减少2级);所有患者均未见肝肾功能损害等毒副反应,无管道滑脱及腹腔感染现象.结论腹膜假粘液瘤行腹腔热灌注化疗联合深部热疗近期临床疗效显著,可有效降低复发率,显著提高患者生存质量.Objective To explore the short-term effects of hyperthermic intraperitoneal perfusion chemotherapy combined with deep hyperthermia in patients with pseudomyxoma of the peritoneum,and to summarize the nursing points and experience of hyperthemiic chemotherapy.Methods The clinical data of 47 patients with pseudomyxoma of the peritoneum who underwent cisplatin(35~40 mg/m^(2))intraperitoneal hyperthermic perfusion chemotherapy combined with deep hyperthenni.i were retrospecrively analyzed,targeted nursing measures were taken during the treatment,and recent results were counted and poor observarions were made,reacrion.Results All 47 patients successflilly completed c isplatin intraperitoneal hyperthermia chemotherapy combined with deep hyperthermia>2 courses,of which 6 were complete remission(CR),28 were partial remission(PR),and 8 were stable(SD).The effective rate was 72.3%,the clinical benefit rate(CBR)was 89.4%.Twenty-one patients developed gastrointestinal reactions of grade I to II on the next day after treatment,manifested as mild nausea and vomiting,and decreased appetite;5 patients developed bone marrow suppression(leukopenia grade 2)3 weeks after treatment;no liver was seen in all parients toxic and side effects such as renal damage,no tube slippage and abdominal infection.Conclusion Peritoneal pseudomyxoma treated with intraperitoneal hyperthermic perflision chemotherapy combined with deep hyperthennia has significant short-term clinical effect,which can effecrively reduce the recurrence rate and significandy improve the patient's quality oflife.
