摘　　要：目的:探讨用Ahmed青光眼阀后房植入术治疗眼外伤继发青光眼的效果。方法:选取2018年12月至2020年1月期间四川省绵阳市中心医院收治的16例(16只眼)眼外伤继发青光眼患者作为研究对象。这些患者入院后,均对其进行Ahmed青光眼阀后房植入术。手术后,对这些患者均随访6个月,比较在手术前及手术后的不同时间这些患者的眼压、并发症的发生率及手术的总有效率。结果:这些患者手术的总有效率为93.75%。在手术后1天、1周、1个月、3个月、6个月时,这些患者的眼压均低于手术前,P<0.05。在手术后的第1天,在这些患者中,出现前房积血、低眼压合并脉络膜脱离的患者各有1例,所占的比例为12.5%。在手术后4个月,在这些患者中,有2例患者(占12.5%)的阀体被周围纤维组织瘢痕化包裹,眼压升高。在随访期间,这些患者均未发生感染、角膜内皮失代偿、青光眼阀移位或暴露的并发症。结论:用Ahmed青光眼阀后房植入术治疗眼外伤继发青光眼的效果较为理想,安全性较高。Objective:To investigate the effect of Ahmed glaucoma valve posterior chamber implantation in the treatment of glaucoma secondary to ocular trauma.METHODS:A total of 16 patients(16 eyes)with glaucoma secondary to eye trauma admitted to Mianyang Central Hospital of Sichuan Province from December 2018 to January 2020 were selected as the research subjects.After admission,all patients underwent AHMED glaucoma valve posterior chamber implantation.After the operation,these patients were followed up for 6 months,and the intraocular pressure,the incidence of complications and the total effective rate of the operation were compared before and after the operation.Results:The total effective rate of these patients was 93.75%.At 1 day,1 week,1 month,3 months and 6 months after operation,the IOP of these patients was lower than that before operation(P<0.05).On the first day after surgery,there was 1 case of anterior chamber hematocele,1 case of low intraocular pressure and 1 case of choroidal detachment,accounting for 12.5% of the patients.Four months after surgery,in two of these patients(12.5%),the valve body was scarred by surrounding fibrous tissue and the IOP was elevated.None of these patients had complications of infection,corneal endothelium decompensation,glaucoma valve displacement,or exposure during follow-up.Conclusion:Ahmed glaucoma valve posterior chamber implantation is effective and safe in the treatment of glaucoma secondary to ocular trauma.
