通脉睛明汤联合康柏西普玻璃体腔注射治疗视网膜静脉阻塞继发黄斑水肿的疗效及对房水VEGF、bFGF、SDF-1的影响  

Effects of Tongmai Jingming Decoction Combined with Conbercept by Intravitreal Injection in the Treatment of Macular Edema Secondary to Retinal Vein Occlusion and Its Influence on VEGF,bFGF and SDF-1 in Aqueous Humor

摘　　要：目的:观察通脉睛明汤联合康柏西普玻璃体腔注射治疗视网膜静脉阻塞继发黄斑水肿的疗效,及对患者房水血管内皮生长因子(VEGF)、碱性纤维母细胞生长因子(bFGF)、人基质细胞衍生因子-1(SDF-1)水平的影响。方法:125例视网膜静脉阻塞继发黄斑水肿患者随机分为研究组75例和对照组50例。对照组给予康柏西普眼用注射液0.05 ml玻璃体腔内注射,每月1次,连续用药3个月;研究组在对照组基础上加用通脉睛明汤治疗,每日1剂,bid,共治疗3周。比较两组患者治疗前后房水VEGF、bFGF、SDF-1水平和血清IL-6、IL-8水平,以及眼部最佳矫正视力(BCVA)、黄斑中心凹厚度(CMT)变化情况,观察两组疗效与并发症发生情况。结果:研究组总有效率为98.67%,高于对照组的88.00%(P<0.05)。治疗后,两组患者的房水VEGF、SDF-1水平和血清IL-6、IL-8水平及眼部BCVA、CMT水平均较治疗前显著下降(P<0.05),且研究组上述指标均明显低于对照组(P<0.05)。研究组并发症发生率为4.00%,显著低于对照组的16.00%(P<0.05)。结论:通脉睛明汤联合康柏西普玻璃体腔注射治疗视网膜静脉阻塞继发黄斑水肿疗效较好,可降低患者房水VEGF、SDF-1水平和血清IL-6、IL-8水平,且安全性较高,值得临床推广。Objective:To observe the efficacy of Tongmai Jingming decoction combined with conbercept by intravitreal injection in the treatment of macular edema secondary to retinal vein occlusion,and the treatment of aqueous vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and stromal cell-derived factor-1(SDF-1)levels in aqueous humor.Methods:Totally 125 patients with macular edema secondary to retinal vein occlusion were randomly divided into study group(75 cases)and control group(50 cases).The control group was treated with conbercept ophthalmic injection 0.05 ml by intravitreal injection once a month,for 3 months;the study group was treated with 1 dose of Tongmai Jingming decoction per day on the basis of the control group,2 times a day,1 week was a course of treatment,3 courses in total.The levels of VEGF,bFGF,SDF-1 in aqueous humor,IL-6,IL-8 in serum,best corrected visual acuity(BCVA),central macular thickness(CMT)in eyes of the two groups before and after treatment were observed,and the efficacy and complications of the two groups were observed.Results:The total effective rate of the study group was 98.67%,which was higher than the 88.00%of the control group(P<0.05).After treatment,the levels of VEGF,SDF-1 in aqueous humor,IL-6,IL-8 in serum,BCVA and CMT in eyes of both groups were lower than those before treatment(P<0.05),and the above indicators of the study group were significantly lower than the control group(P<0.05).The complication rate in the study group was 4.00%,which was significantly lower than the 16.00%in the control group(P<0.05).Conclusion:Tongmai Jingming decoction combined with conbercept by intravitreal injection has a better therapeutic effect on macular edema secondary to retinal vein occlusion,it can reduce the levels of VEGF,bFGF,SDF-1 in aqueous humor and IL-6,IL-8 in serum,and has high safety,which is worthy of clinical promotion.
