摘　　要：目的评估卒中中心对急性缺血性脑卒中(acute ischemic stroke)患者进行团队治疗对临床疗效的影响.方法选择2017年1月—2017年6月急性缺血性脑卒中患者100例,根据随机对照法分为对照组(n=48)和观察组(n=52),观察组进行团队治疗(多学科诊疗),对照组进行基础用药疗法.出院后或转科后予以康复锻炼,两组均治疗1个月.评估两组治疗后10 d、30 d的日常生活活动能力和神经功能缺损情况.比较2017年1月—2017年12月卒中中心治疗团队成立后和2016年1月—2016年12月卒中中心团队成立前的吞咽困难筛查情况、住院第2日末的抗血栓治疗、深静脉血栓(DVT)预防情况.结果观察组治疗后10 d、30 d的日常生活活动能力评分和神经功能缺损评分显著优于对照组(P<0.01);2017年卒中中心治疗团队成立后的吞咽困难筛查情况、住院第二日末的抗血栓治疗、深静脉血栓(DVT)预防情况与2016年卒中中心团队成立前比较显著改善(P<0.01).结论卒中中心团队治疗急性缺血性脑卒中能明显提高临床疗效,值得在临床上推广应用.Objective To evaluate the impact of comprehensive stroke management team in stroke center on the clinical efficacy of acute ischemic stroke(AIS)patients.Methods A total of 100 AIS patients treated from January 2017 to June 2017 were selected and randomly assigned to control group(n=48)and observation group(n=52).The observation group was treated by comprehensive stroke management team;the control group was treated with drug therapy and rehabilitation exercise after discharge from hospital or transfer to the other departments.After 30d of treatment,the activity of daily living and neurologic impairments after 10d and 30d of treatment were evaluated.The rate of taking the dysphagia screening,anti-thrombotic therapy and deep venous thrombosis(DVT)prevention before establishment of comprehensive stroke management team(January 2016-December 2016)and after establishment of comprehensive stroke management team(January 2017-December 2017)was compared.Results After 10d and 30d of treatment,the activity of daily living and neurologic impairments in the observation group were better than those of control group,the difference was statistically significant(P<0.05);the rate of taking the dysphagia screening,anti-thrombotic therapy and DVT prevention in 2017 was higher than that in 2016,the difference was statistically significant(P<0.05).Conclusion The comprehensive stroke management team can significantly improve the clinical efficacy of stroke treatment and is worthy of clinical promotion.
