环扎法致大鼠脊髓损伤减压后TGF-β1表达与凋亡细胞的相关性

摘  要:目的研究大鼠脊髓损伤不同时间点减压后脊髓损伤段标本TGF-β1的表达和凋亡细胞的变化,以及不同时间点减压后大鼠神经功能恢复情况。方法70只SD雄性大鼠随机分成5组,麻醉后手术切除胸椎10-11椎板,暴露脊髓。假手术组10只,行胸椎10-11椎板切除,仅暴露脊髓,不环扎。其余4组作为手术组,每组15只,暴露脊髓后,用环扎法使硬膜囊截面压缩至原截面积的70%,建立截瘫模型、完成此次建模手术,减压组术后分别在环扎8,24,72 h后进行二次手术,去除环扎线减压,命名为8 h减压组、24 h减压组和72 h减压组;不减压组,环扎后不进行二次手术减压。假手术组和不减压组只进行一次手术,在术后1,3,7,14,21 d统计神经功能评分并处死大鼠取出脊髓。8 h减压组、24 h减压组和72 h减压组二次手术后1,3,7,14,21 d统计神经功能评分并处死大鼠,取出脊髓。各组脊髓标本切片行HE染色观察脊髓组织病理变化,免疫组化测定各时间点TGF-β1的表达,TUNEL法计数凋亡细胞,功能评分数据分析神经功能恢复情况。结果假手术组大鼠标本各取材时间点的实验切片均未见明显异常。与假手术组相比,手术组TGF-β1阳性细胞数及神经细胞凋亡细胞数于术后1 d均有增加,术后3 d达高峰,在术后7 d表达逐渐减弱,至术后21 d仍有表达(P<0.05)。72 h减压组及不减压组两组大鼠TGF-β1阳性细胞数及凋亡细胞数普遍高于早期减压的8 h减压组及24 h减压组,差异有统计学意义(P<0.05)。结论脊髓损伤后TGF-β1的表达增加,可能对凋亡有阻断作用,可减轻脊髓损伤的范围。环扎法制作脊髓损伤动物模型是一种理想的造模方法,早期减压对减轻脊髓的损害及促进神经功能的恢复有一定的意义。Objective To investigate the expression of TGF-β1 and the apoptotic cells in spinal cord injury specimens at different time points after decompression in rats and the recovery of neurological function at different time points after decompression.Methods Seventy male SD rats were randomly divided into 5 groups,and all rats were performed surgical resection of thoracic lamina 10-11 after anesthesia to expose the spinal cord.Ten rats in sham operation group underwent thoracic laminectomy 10-11,and only the spinal cord was exposed without cerclage.In the other four groups(n=15 in each group),the section of the dural sac was compressed to 70%of the original cross-sectional area with the cerclage method after the spinal cord was exposed,and then the paraplegia model was established.The second operation was performed to remove the ring binding line according to the cerclage time after surgery,named as 8 h decompression group,24 h decompression group,72 h decompression group,and non-decompression group.In non-decompression group,only ring binding was performed but no second operation decompression.In sham operation group and non-decompression group,the neurological function scores were recorded at days 1,3,7,14,21 after the first operation,and then the rats were sacrificed to collect the spinal cord.In 8 h decompression group,24 h decompression group and 72 h decompression group,the neurological function scores were recorded at days 1,3,7,14,21 after the second operation,and then the rats were sacrificed to collect the spinal cord.The pathological changes of spinal cord tissue were observed by HE staining,the expression of TGF-β1 was measured by immunohistochemistry at each time point,apoptosis cells were counted by TUNEL method,and neurological function recovery was analyzed by functional score data.Results In sham operation group,no obvious abnormality was found in the experimental sections at each time point.Compared with sham operation group,the numbers of TGF-β1 positive cells and neuronal apoptotic cells in four
