浅析术中预防性截囊技术对糖尿病患者白内障术后囊袋收缩的预防效果

摘　　要：目的研究白内障手术中预防性截囊技术对糖尿病患者术后囊袋收缩的影响。方法本次研究选取2018年1月至2019年6月至眼科进行了白内障超声乳化吸除联合人工晶状体植入手术,并患有糖尿病的白内障患者共67例(67只眼),随机分为观察组和对照组,其中,观察组34例,对照组33例,观察组患者于术中植入人工晶状体后,采取了预防性截囊技术,对照组仅接受常规手术流程,术后观察6个月,就两组患者前房炎症反应、前囊膜收缩情况、人工晶状体位置变化及是否发生后发性白内障等几个方面情况做对比观察。结果观察组在术后第1天和第1周的炎症反应情况和对照组相比无明显差异,不具有统计学意义(P>0.05),两组患者术后观察第6个月的前囊膜收缩情况相比,观察组总发生率为(70.59%)比对照组(84.85%)低,组间数据有差异,具有统计学意义(P<0.05);术后两组患者的人工晶状体位置均未发生明显改变,后发性白内障发生频率数据差异也无统计学意义(P>0.05)。结论在术中为糖尿病患者植入人工晶状体后进行预防性截囊技术,可以有效预防这类患者在白内障术后发生囊袋收缩的情况,同时也不会增加患者发生炎症反应和后发性白内障等并发症的几率,可以用于临床医疗治疗中。Objective To study the effect of prophylactic capsulotomy on capsular contraction in diabetic patients after cataract surgery.Methods From January 2018 to June 2019,phacoemulsification combined with intraocular lens implantation was performed in the ophthalmology department,67 cataract patients (67 eyes) with diabetes mellitus were randomly divided into observation group and control group,including 34 cases in the observation group and 33 cases in the control group.After intraoperative intraocular lens implantation,the observation group adopted preventive capsulotomy.The control group only received routine procedure.After 6 months of observation,the two groups of patients with anterior chamber inflammation,anterior capsular contraction,intraocular lens position changes and whether there were posterior cataract and other aspects were compared.Results Compared with the control group,the inflammatory reaction of the observation group on the first day and the first week after operation had no significant difference (P>0.05).The total incidence rate of the observation group (70.59%) was lower than that of the control group (84.85%),and the difference between the two groups was statistically significant (P<0.05).There was no significant difference in the intraocular lens position between the two groups,and there was no significant difference in the frequency of posterior cataract between the two groups (P>0.05).Conclusion Prophylactic capsulotomy for diabetic patients after intraoperative intraocular lens implantation can effectively prevent the capsular contraction of these patients after cataract surgery,and it will not increase the incidence of complications such as inflammatory reaction and posterior cataract,which can be used in clinical medical treatment.
