血清脂联素、血浆NSE与急性脑梗死的相关性研究

摘　　要：目的探讨血清脂联素、血浆NSE与急性脑梗死的相关性。方法选取本院2018年9月至2020年1月收治的350例急性脑梗死住院患者为观察对象,另随机选取350例同期在我院体检中的健康者为对照组,按神经功能缺损程度将患者分为轻、中、重型,按梗死灶面积将患者分为小、中、大病灶组,比较不同神经功能缺损患者与健康者的血清脂联素和血浆NSE水平,不同梗死灶面积患者与健康者的血清脂联素和血浆NSE水平。结果重型患者的血清脂联素水平明显低于中型患者,轻型患者的血清脂联素水平最高,不同程度神经功能缺损的急性脑梗死患者的血清脂联素水平均明显低于健康者;重型患者的血浆NSE水平明显高于中型患者,轻型患者的血浆NSE水平最低,不同程度神经功能缺损的急性脑梗死患者的血浆NSE水平均明显高于健康者;不同程度神经功能缺损患者的各指标间差异具有统计学意义,P<0.05。大梗死灶患者的血清脂联素水平明显低于中梗死灶患者,小梗死灶患者的血清脂联素水平最高,不同面积梗死灶的急性脑梗死患者的血清脂联素水平明显低于健康者;大梗死灶患者的NSE水平明显高于中梗死灶患者,小梗死灶患者的NSE水平最低,不同面积梗死灶的急性脑梗死患者的NSE水平明显高于健康者;不同面积梗死灶患者的各指标间差异具有统计学意义,P<0.05。结论脂联素和血浆NSE水平与急性脑梗死的发生发展存在密切关系,脂联素水平依据神经功能缺损的不同程度呈现下降趋势,在不同梗死灶面积患者中,梗死灶越大,脂联素水平越低,血浆NSE水平依据神经功能缺损的不同程度呈现升高趋势,在不同梗死灶面积患者中,梗死灶越大,血浆NSE水平越高,测定脂联素和血浆NSE水平可对急性脑梗死的诊断与治疗提供有力依据,值得临床推广应用。Objective To investigate the correlation between serum adiponectin,plasma NSE and acute cerebral infarction.Methods 350 patients with acute cerebral infarction admitted to our hospital from September 2018 to January 2020 were selected as the observation objects.Another 350 healthy patients in our hospital during the same period were randomly selected as the control group.The patients were divided into light,medium and severe patients according to the degree of neurological impairment,and the patients were divided into small,medium and large lesion groups according to the area of infarction.Serum adiponectin and plasma NSE levels were compared between patients with different neurological defects and healthy patients.Serum adiponectin and plasma NSE levels were compared between patients with different infarcts areas and healthy subjects.Results Serum adiponectin levels were significantly lower in severe patients than those in moderate patients,mild patients had the highest serum adiponectin levels,serum adiponectin levels in patients with acute cerebral infarction with different degrees of neurological impairment were significantly lower than those in healthy patients.The plasma NSE level of the severe patients was significantly higher than that of the medium-sized patient.Mild patients had the lowest levels of NSE.The plasma NSE level of patients with acute cerebral infarction with different degree of neurological impairment was significantly higher than that of healthy patients.The differences among the indexes of patients with different degree of neurological impairment were statistically significant,P<0.05.Serum adiponectin levels in patients with large infarcts were significantly lower than those with medium infarcts.Serum adiponectin levels were highest in patients with small infarcts.Serum adiponectin levels were significantly lower in patients with acute cerebral infarction of different sizes than those in healthy patients.The NSE level of patients with large infarction was significantly higher than that of
