尿BLCA-1、Haase水平在膀胱癌诊断及术后复发监测中的应用价值

摘　　要：目的探讨尿膀胱特异性核基质蛋白-1(BLCA-1)、透明质酸酶(HAase)水平在膀胱癌诊断及术后复发监测中的应用价值。方法选取80例膀胱癌患者作为膀胱癌组,80例前列腺增生患者作为前列腺增生组,80例健康志愿者作为对照组,检测三组研究对象尿液中的BLCA-1、HAase水平,并分析BLCA-1、HAase的诊断效能。结果三组受检者的尿BLCA-1、HAase水平比较:膀胱癌组>前列腺增生组>对照组,且两两组间比较,差异均有统计学意义(P<0.05)。BLCA-1、HAase及二者联合诊断膀胱癌的AUC分别为0.921、0.866、0.934;灵敏度分别为80.0%、98.8%、90.0%;特异度分别为95.0%、70.0%、85.6%。术后1年,膀胱癌复发患者尿BLCA-1、HAase水平高于术前,且复发患者高于未复发患者(P<0.05)。结论膀胱癌及术后复发患者的BLCA-1、HAase水平较高,二者联合诊断膀胱癌的准确度高,可成为诊断膀胱癌、监测术后复发的重要指标。Objective To explore the application value of urinary bladder can cerspecific antigen-1(BLCA-1)and hyaluronidase(HAase)levels in diagnosis of bladder cancer and postoperative recurrence monitoring.Methods A total of 80 patients with bladder cancer were selected as bladder cancer group,80 patients with prostate hyperplasia as prostate hyperplasia group,and 80 healthy volunteers as control group.The levels of urinary BLCA-1 and HAase of the three groups were measured,and the diagnostic efficacy of BLCA-1 and HAase were analyzed.Results The levels of urinary BLCA-1 and HAase in the three groups were compared:the bladder cancer group>the prostate hyperplasia group>the control group,and the differences were statistically significant between the two groups(P<0.05).The AUC of BLCA-1,HAase and the combination of the two in the diagnosis of bladder cancer were 0.921,0.866 and 0.934 respectively;the sensitivity were 80.0%,98.8%and 90.0%respectively;the specificity were 95.0%,70.0%and 85.6%respectively.One year after operation,the levels of urinary BLCA-1 and HAase in patients with recurrent bladder cancer were higher than those before operation,and those in the patients with recurrent bladder cancer were higher than the patients without recurrence(P<0.05).Conclusion The levels of BLCA-1 and HAase in patients with bladder cancer and postoperative recurrence are higher.The combined diagnosis of BLCA-1 and HAase has high accuracy,which can be an important indicator for the diagnosis of bladder cancer and monitoring the recurrence after operation.
