活血化瘀中药在胃癌防治中的研究进展

摘　　要：胃癌是我国常见的上消化道恶性肿瘤之一,其早期缺乏特异性症状和体征,具有高发病率和死亡率特点。胃癌现有的临床治疗方式包括手术、化疗、靶向治疗和免疫治疗,但存在早期发现困难、副作用大、耐药和治疗费用昂贵等问题。血瘀是肿瘤的典型病理特征,活血化瘀是中医药肿瘤治疗中的重要方法,在提高患者免疫力和改善生活质量方面具有独特的优势。丹参、莪术、川芎、温郁金等活血化瘀单味中药和四君子汤、香砂六君子汤、膈下逐淤汤、柴胡疏肝散等具有活血化瘀作用的中药复方,对缩短胃癌术后创面恢复时间、抑制胃癌细胞增殖、诱导细胞凋亡和自噬、抑制胃癌细胞耐药和提高机体免疫力等方面具有重要作用。本文总结了胃癌的中医基础理论研究现状,并对活血化瘀单味中药和复方药物的抗胃癌研究进行了综述,以期为中医临床防治胃癌提供参考借鉴,加快活血化瘀中药的抗胃癌研发进程。Gastric cancer is one of the most common malignant tumors of the upper digestive tract in China.It lacks specific symptoms and signs at early stage and has high morbidity and mortality.The existing clinical treatment methods for gastric cancer include surgery,chemotherapy,targeted therapy and immunotherapy,but there are some problems such as difficulty in early detection,great side effects,drug resistance and expensive treatment cost.Blood stasis is a typical pathological feature of tumor,and promoting blood circulation and removing blood stasis is an important method in the treatment of tumor with traditional Chinese medicine,which has unique advantages in improving immunity and quality of life of patients.Salvia miltiorrhiza,zedoary turmeric,Chuanxiong,Wenyujin and other traditional Chinese medicine with the function of promoting blood circulation and removing blood stasis,and Sijunzi decoction,Xiangsha Liujunzi decoction,Gexia Zhuyu decoction,Chaihu Shugan powder and other traditional Chinese medicine compounds with the function of promoting blood circulation and removing blood stasis,plays an important role in shortening the wound healing time,inhibiting the proliferation of gastric cancer cells,inducing apoptosis and autophagy,inhibiting drug resistance of gastric cancer cells and improving the body immunity.This paper summarized the current research status of the basic theory of traditional Chinese medicine for gastric cancer,and reviewed the anti-gastric cancer research of single Chinese medicine and compound drugs for promoting blood circulation and removing blood stasis,in order to provide reference for the clinical prevention and treatment of gastric cancer by traditional Chinese medicine,and accelerate the anti-gastric cancer research and development process of traditional Chinese medicine for promoting blood circulation and removing blood stasis.
