摘  要:目的观察双微导管技术及可视化腔内支撑装置(LVIS支架)辅助弹簧圈栓塞术治疗急性期颅内宽颈动脉瘤的疗效,探讨其对患者血清可溶性细胞间黏附分子-1(SICAM-1)与神经功能的影响。方法选择2018年9月至2020年10月茂名市中医院外二科收治的50例急性期颅内宽颈动脉瘤患者为研究对象。采用随机数表法分为观察组和对照组,每组25例。观察组采用双微导管技术治疗,对照组采用LVIS支架辅助弹簧圈栓塞术治疗,手术结束时比较两组患者的治疗效果及简易精神状态检查量表(MMSE)评分,同时比较两组患者术前、术后的SICAM-1和中枢神经特异性蛋白(S100β)水平和术后并发症发生情况。结果观察组和对照组患者的治疗总有效率分别为96.0%、92.0%,差异均无统计学意义(P>0.05);观察组和对照组患者手术结束时的各项MMSE评分比较差异无统计学意义(P>0.05);术后,两组患者的SICAM-1、S100β表达水平明显下降,且观察组分别为(142.32±13.89)mg/L、(1.89±0.31)mg/L,明显低于对照组的(189.74±14.76)mg/L、(2.21±0.42)mg/L,差异均有统计学意义(P<0.05);观察组患者术后并发症发生率为8.0%,明显低于对照组的32.0%,差异有统计学意义(P<0.05)。结论双微导管技术与LVIS支架辅助栓塞术治疗急性期颅内宽颈动脉瘤效果相当,但双微导管技术更能有效调节血清SICAM-1、S100β表达水平,降低术后并发症。Objective To observe the curative effect of double microcatheter technique and LVIs stent assisted coil embolization in the treatment of acute intracranial wide-necked aneurysms,and to explore the effect on serum soluble intercellular adhesion molecule-1(sICAM-1)and nerve function.Methods Fifty patients with acute intracranial wide-necked aneurysms admitted to Second Department of Surgery,Maoming Hospital of Traditional Chinese Medicine from September 2018 to October 2020 were selected as the research objects.They were randomly divided into observation group and control group,with 25 patients in each group.The observation group was treated with double microcatheter technology,and the control group was treated with LVIs stent assisted coil embolization.At the end of operation,the treatment effect and MMSE score of the two groups were compared,and the levels of sICAM-1 and S100βbefore and after operation and the incidence of postoperative complications were compared.Results The total effective rates of the observation group and the control group were 96.0%and 92.0%,with no statistically significant difference(P>0.05);the MMSE scores of the observation group and the control group at the end of operation had no significant difference(P>0.05);after operation,the expression levels of sICAM-1 and S100βin the two groups were significantly decreased,and the expression levels of sICAM-1 and S100βin the observation group were(142.32±13.89)mg/L and(1.89±0.05)mg/L,significantly lower than(189.74±14.76)mg/L,(2.21±0.42)mg/L in the control group(P<0.05);the incidence of postoperative complications in the observation group was 8.0%,significantly lower than 32.0%in the control group(P<0.05).Conclusion Double microcatheter technique and LVIs stent assisted embolization have the same effect in the treatment of acute intracranial wide-necked aneurysms,which can effectively improve the cognitive function of patients,but the double microcatheter technology can more effectively regulate the expression levels of sICAM-1 and S100β
