摘 要:目的比较锥光束乳腺CT(CBBCT)和乳腺MRI对乳腺良、恶性病变的诊断效能。资料与方法收集接受乳腺CBBCT和(或)MRI检查并取得病灶病理结果的患者38例,其中32例完成2项检查,以病理结果为“金标准”评价2种方法的诊断正确率,并比较2种检查方法的影像特征表现差异。结果CBBCT与MRI总的检查诊断正确率分别为84.2%(32/38)、87.5%(28/32),差异无统计学意义(P>0.05);CBBCT与MRI对乳腺恶性病灶的诊断正确率分别为92.6%(25/27)、100%(22/22),差异无统计学意义(P>0.05);CBBCT与MRI对乳腺良性病灶的诊断准确率分别为63.6%(7/11)、60.0%(6/10),差异无统计学意义(P>0.05)。2种检查方法在乳腺腺体类型、病变部位、类型及肿块形态、边缘、内部强化特征和大小测量方面差异无统计学意义(P>0.05)。结论CBBCT与MRI在影像特征(乳腺纤维腺体类型、病变类型、形态、边缘及内部强化特征)、病变范围测量方面具有较高的一致性,诊断乳腺良、恶性病变有较高的准确性。Purpose To compare the diagnostic efficacy of cone-beam breast CT(CBBCT)and breast MRI in benign and malignant breast lesions.Materials and Methods Thirty-eight patients underwent CBBCT and/or MRI examination and obtained pathological results.Thirty-two patients completed both the two examinations.The diagnostic accuracy of the two methods was evaluated according to the pathological results as the gold standard,and the differences of imaging features were compared between the two examinations.Results The diagnostic accuracy of CBBCT and MRI was 84.2%(32/38)and 87.5%(28/32),respectively;with no significant difference(P>0.05),and the diagnostic accuracy of malignant breast lesions was 92.6%(25/27)and 100%(22/22)respectively;with no significant difference(P>0.05).The diagnostic accuracy of benign breast lesions was 63.6%(7/11)and 60.0%(6/10)respectively,with no significant difference(P>0.05).There was no significant difference between the two methods in breast gland type,lesion location,type,mass shape,edge,internal enhancement characteristics and size measurement(P>0.05).Conclusion CBBCT and MRI have high consistency in the measurement of imaging features and lesion range,and have high accuracy in the diagnosis of benign and malignant breast lesions.CBBCT is an ideal method for diagnosis of breast lesions,which is similar to that of MRI.
