奥沙利铂联合贝伐珠单抗治疗结肠癌的疗效及对患者免疫功能的影响研究

摘　　要：目的探讨奥沙利铂联合贝伐珠单抗治疗结肠癌的疗效及对患者免疫功能的影响。方法98例结肠癌患者,随机分为对照组和观察组,各49例。对照组采用XELOX方案治疗,观察组在对照组基础上使用贝伐珠单抗治疗。对比两组临床疗效、治疗前后T淋巴细胞亚群水平、毒副反应发生率。结果观察组客观缓解率(RR)为69.39%、疾病控制率(DCR)为91.84%,均明显高于对照组的42.86%、75.51%,差异有统计学意义(P<0.05)。治疗后,观察组CD3^(+)、CD4^(+)、CD8^(+)、CD3^(+)CD8^(+)、CD3^(+)CD56^(+)、NKT、CD4^(+)CD25^(+)水平均明显高于对照组,CD3^(+)CD4^(+)水平明显低于对照组,差异有统计学意义(P<0.05)。观察组毒副反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论奥沙利铂联合贝伐珠单抗治疗结肠癌的效果显著,有助于增强抗肿瘤效果,改善免疫抑制状态,有效延长生存时间,提升生存质量,且不增加毒副反应,具有积极的临床意义。Objective To discuss the efficacy of oxaliplatin combined with bevacizumab on immune function in the treatment of colon cancer and its influence on the immune function of patients.Methods A total of 98 patients with colon cancer were randomly divided into control group and observation group,with 49 cases in each group.The control group was treated with XELOX regimen,and the observation group was treated with bevacizumab based on the control group.The clinical efficacy,T lymphocyte subsets before and after treatment,and incidence of side effects were compared between the two groups.Results The response rate(RR)and disease control rate(DCR)of the observation group were 69.39%and 91.84%,which were obviously higher than 42.86%and 75.51%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD8^(+),CD3^(+)CD8^(+),CD3^(+)CD56^(+),NKT and CD4^(+)CD25^(+)of the observation group were obviously higher than those of the control group,and CD3^(+)CD4^(+)level was obviously lower than the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of side effects between the observation group and control group(P>0.05).Conclusion Combination of oxaliplatin and bevacizumab shows significant effect in the treatment of advanced colon cancer,which can help enhance the anti-tumor effect,improve the state of immune suppression,prolong the survival time,and improve the quality of life without increase of side effects.It has a positive clinical significance.
