水平肌加强减弱术与下斜肌减弱术联合治疗对儿童斜视临床效果的影响

摘　　要：目的:探讨水平肌加强减弱术与下斜肌减弱术联合治疗儿童斜视的临床效果。方法:回顾分析本院2016年4月-2018年12月收治的儿童斜视60例,按治疗方式分对照组(30例,接受水平肌加强减弱术治疗)和研究组(30例,接受水平肌加强减弱术联合下斜肌减弱术治疗)。比较两组治疗效果、治疗前后斜视度数、围术期指标、并发症。结果:研究组治疗总有效率(93.33%)高于对照组(70.00%),差异有统计学意义(P<0.05)。治疗前两组斜视度数比较差异无统计学意义(P>0.05),研究组术后1、3、6个月斜视度数均低于对照组(P<0.05)。两组手术时间比较差异无统计学意义(P>0.05),研究组术中出血量、视力恢复时间优于对照组(P<0.05)。研究组并发症发生率为6.67%,对照组为10.00%,差异无统计学意义(P>0.05)。结论:临床治疗儿童斜视可考虑给予水平肌加强减弱术联合下斜肌减弱术治疗,效果突出。Objective:To investigate the clinical effect of horizontal muscle strengthening and weakening combined with inferior oblique muscle weakening in the treatment of strabismus in children.Method:A total of 60 cases of children with strabismus treated in our hospital from April 2016 to December 2018 were analyzed retrospectively,and they were divided into the control group (30 cases,received the horizontal muscle strengthening and weakening operation) and the study group (30 cases,received the horizontal muscle strengthening and weakening combined with inferior oblique muscle weakening operation).The therapeutic effect,degree of strabismus before and after treatment,perioperative indexes,complications of the two groups were compared.Result:The total effective rate of the study group (93.33%) was higher than that of the control group (70.00%),and the difference was statistically significant (P<0.05).There was no difference in the strabismus degree between the two groups before treatment (P>0.05);the degree of strabismus after operation for 1,3 and 6 months in the study group was lower than that in the control group (P<0.05).There was no significant difference in the operation time between the two groups (P>0.05).The amount of intraoperative bleeding and visual acuity recovery time in the study group were better than those in the control group (P<0.05).The incidence of complications in the study group was 6.67%,that in the control group was 10.00%,there was no significant difference (P>0.05).Conclusion:The clinical treatment of children with strabismus can be considered that give the horizontal muscle strengthening and weakening operation combined with the inferior oblique muscle weakening operation,which the clinical effect are outstanding.
