摘  要:目的:探讨乳腺癌不同YEATS域含蛋白质4(YEATS4)和SOX基因家族的转录因子SOX12表达状态的常规超声、剪切波弹性成像(SWE)和超声造影(CEUS)征象,并分析YEATS4、SOX12表达状态与多模态超声特征的相关性。方法:纳入行手术治疗的210例乳腺癌女性患者为研究对象,术前进行常规超声、SWE和CEUS检查,采用免疫组织化学法检测YEATS4、SOX12的表达状态。采用单因素分析和多因素Logistic回归分析多模态超声特征与YEATS4、SOX12表达状态的相关性。结果:120例(57.14%)YEATS4阳性表达,90例(42.86%)阴性表达;128例(60.95%)SOX12阳性表达,82例(39.05%)阴性表达。对于常规超声特征,YEATS4阳性乳腺癌患者出现肿块形态不规则、肿块边界模糊和微小钙化的比例显著高于YEATS4阴性者(P值均<0.05);SOX12阳性乳腺癌患者出现肿块形态不规则、微小钙化和2~3级血流分级的比例显著高于SOX12阴性者(P值均<0.05)。对于SWE征象,YEATS4阳性乳腺癌患者出现硬环征和黑洞征的比例均显著高于YEATS4阴性者(P值均<0.05);SOX12阳性乳腺癌患者出现硬环征的比例显著高于SOX12阴性者(χ^(2)=20.430,P<0.001)。对于CEUS征象,YEATS4阳性乳腺癌患者出现高增强、灌注缺损的比例显著高于YEATS4阴性者(P值均<0.05);SOX12阳性乳腺癌患者出现高增强和周围放射状增强的比例显著高于SOX12阴性者(P值均<0.05)。多因素Logistic回归分析结果显示,肿块形态不规则、伴微小钙化、出现硬环征及CEUS表现为高增强是YEATS4阳性表达的独立危险因素(P值均<0.05);肿块形态不规则、出现硬环征、CEUS表现为周围放射状增强是SOX12阳性表达的独立危险因素(P值均<0.05)。结论:乳腺癌多模态超声特征可用于预测YEATS4、SOX12的表达状态,从而有助于治疗方案的选择及术前无创性评估患者预后。Objective:To investigate the conventional ultrasound,shear wave elastography(SWE)and contrast-enhanced ultrasonography(CEUS)manifestations of different YEATS4 expression states in Yeats domain-containing protein 4 and SOX12 expression states in SOX(SRY box-containing)gene families in breast cancer,and to analyze the correlation between the expression states of YEATS4 and SOX12 and the multimodal ultrasonic characteristics.Methods:A total of 210 female patients with breast cancer who received surgical treatment were included in this study.Routine ultrasound,SWE and CEUS were performed preoperatively,and the expression of YEATS4 and SOX12 was detected by immunohistochemistry.Univariate analysis and multivariate Logistic regression were used to analyze the relationship between multimodal ultrasonic characteristics and the expression states of YEATS4 and SOX12.Results:120 cases(57.14%)had positive expression of YEATS4,while 90 cases(42.86%)had negative expression.SOX12 was positively expressed in 128 cases(60.95%)but negatively expressed in 82 cases(39.05%).As for conventional ultrasonographic features,the proportion of irregular shape,fuzzy boundary and microcalcification in YEATS4-positive breast cancer patients was significantly higher than that in YEATS4-negative breast cancer patients(all P<0.05).The proportion of irregular shape,microcalcification and grade 2~3 blood flow in SOX12-positive breast cancer patients was significantly higher than that in SOX12-negative breast cancer patients(all P<0.05).For SWE signs,the proportion of hard ring sign and black hole sign in YEATS4-positive breast cancer patients was significantly higher than that in YEATS4-negative breast cancer patients(all P<0.05).The proportion of hard ring sign in SOX12-positive breast cancer patients was significantly higher than that in SOX12-negative breast cancer patients(χ^(2)=20.430,P<0.001).For CEUS signs,the proportion of hyperenhancement and perfusion defect in YEATS4-positive breast cancer patients was significantly higher than that in
