摘  要:目的探讨核因子E2相关因子(NrF)2/Kelch样环氧氯丙烷相关蛋白(Keap)1/抗氧化反应元件(ARE)信号通路在老年性青光眼患者小梁网组织功能损伤中的作用。方法选取2018年5月至2019年10月确诊为老年性青光眼患者42例(42眼)为老年性青光眼组,以因故死亡成人的供体眼球标本35例为对照组。老年性青光眼组均行小梁切除术,留取小梁网组织标本,对照组切取小梁组织。免疫组化检测标本组织中NrF2、Keap1、ARE阳性表达情况;实时聚合酶链反应(RT-PCR)和Western印迹检测NrF2、Keap1、ARE mRNA和蛋白表达水平;比色法检测过氧化物酶和超氧化物歧化酶水平。结果老年性青光眼组NrF2、ARE阳性表达率明显低于对照组,Keap1阳性率明显高于对照组(P<0.01)。老年性青光眼组小梁网组织中NrF2、ARE mRNA和蛋白表达水平明显低于对照组,Keap1 mRNA和蛋白表达水平明显高于对照组(P<0.05,P<0.01)。老年性青光眼组小梁网组织中过氧化物酶水平明显高于对照组,超氧化物歧化酶水平明显低于对照组(P<0.01)。结论老年性青光眼患者小梁网组织中NrF2、ARE低表达,Keap1高表达,与氧化应激状态有关;激活NrF2/Keap1/ARE信号通路有利于控制患者的病情发展。Objective To investigate the role of nuclear factor E2 related factor(NrF)2/kelch like epichlorohydrin related protein(Keap)1/antioxidant response element(ARE)signaling pathway in trabecular meshwork injury in elderly patients with glaucoma.Methods 42 cases(42 eyes)of senile glaucoma patients diagnosed in hospital from May 2018 to October 2019 were selected as the senile glaucoma group,and 35 cases of donor eyeball samples from adults died for some reason were selected as the control group.Trabeculectomy was performed in the senile glaucoma group,and trabecular meshwork tissue samples were taken,while trabecular tissue samples were taken in the control group.The positive expression of NrF2,Keap1 and ARE were detected by immunohistochemistry.The mRNA and protein expression levels of NrF2,Keap1 and ARE were detected by RT-PCR and Western blot.The levels of peroxidase and superoxide dismutase were detected by colorimetry.Results The positive expression rates of NrF2 and ARE in the senile glaucoma group were significantly lower than those in the control group,and the positive rate of Keap1 in the senile glaucoma group was significantly higher than that in the control group(P<0.01).The mRNA and protein expression levels of NrF2 and ARE in trabecular meshwork of senile glaucoma group were significantly lower than those of control group,and the mRNA and protein expression levels of Keap1 in trabecular meshwork of senile glaucoma group were significantly higher than those of control group(P<0.05).The level of peroxidase〔(103.61±19.38)×103 U/L〕in trabecular meshwork of senile glaucoma group was significantly higher than that of control group〔(72.57±11.24)×103 U/L〕,and the level of superoxide dismutase〔(338.73±31.85)U/L〕was significantly lower than that of control group〔(406.49±38.36)U/L〕(P<0.01).Conclusions Low expressions of NrF2 and ARE and high expression of Keap1 in trabecular meshwork of senile glaucoma patients are related to oxidative stress.Activation of NrF2/Keap1/ARE signaling pathway is be
