甲状腺乳头状癌患者血清miR-21、miR-221、miR-146a水平变化及其与临床特征的关系

摘　　要：目的探讨血清miR-21、miR-221、miR-146a联合检测对甲状腺乳头状癌(PTC)的诊断价值并分析其与临床特征的相关性。方法选择85例甲状腺结节患者根据术后病理学诊断结果分为观察组(PTC)43例和对照组(良性结节)42例。提取所有患者血清样本的总RNA,使用荧光定量PCR检测miR-21、miR-221、miR-146a的表达水平,比较两组患者3种miRNA的表达情况,并分析其表达相关性。比较观察组不同临床特征患者3种miRNA的表达水平,使用受试者工作特征(ROC)曲线分析3项指标单独及联合检测对PTC的诊断价值。结果观察组患者血清miR-21、miR-221表达高于对照组,而miR-146a表达低于对照组(P均<0.05)。观察组肿瘤长径>2 cm、有包膜侵犯、临床分期Ⅲ期、有淋巴结转移患者血清miR-21、miR-221表达水平增加,miR-146a表达水平降低(P均<0.05)。观察组血清miR-21与miR-221表达呈正相关(r=0.738,P<0.05),miR-146a与miR-21及miR-221表达呈负相关(r分别为-0.726、-0.599,P均<0.05)。ROC曲线分析显示,血清miR-21、miR-221、miR-146a诊断PTC的曲线下面积分别为0.790、0.796、0.289,三者联合诊断PTC的曲线下面积为0.914。结论PTC患者血清miR-21、miRNA-22l的表达明显上调,而miR-146a表达水平下降,不同临床特征患者三者血清表达存在差异,三者联合检测可明显增强PTC的诊断价值。Objective To investigate the value of combined detection of serum miR-21,miR-221 and miR-146 a in diagnosing the papillary thyroid carcinoma(PTC)and to analyze their correlation with clinical features.Methods Eightyfive patients with thyroid nodules were divided into the observation group(PTC)of 43 cases and the control group(benign nodules)of 42 cases according to postoperative pathological diagnosis.Total RNA was extracted from serum samples of all patients,and the expression levels of miR-21,miR-221,and miR-146 a were detected by fluorescence quantitative PCR.The expression of three kinds of miRNAs in the two groups were compared,and their correlation was analyzed.The expression levels of three miRNAs with different clinical characteristics in the observation group were compared,and the diagnostic value of the three indicators for PTC was analyzed by receiver operating characteristic(ROC)curve.Results The expression levels of miR-21 and miR-221 in the observation group were higher than those in the control group,while the expression of miR-146 a in the observation group was lower than that in the control group(all P<0.05).In the observation group,the expression levels of miR-21 and miR-221 increased in serum of patients with tumor diameter>2 cm,capsule invasion,clinical stageⅢand lymph node metastasis,while the expression level of miR-146 a decreased(all P<0.05).In the observation group,the expression of miR-21 was positively correlated with miR-221 expression(r=0.738,P<0.05),and the expression of miR-146 a was negatively correlated with the expression of miR-21 and miR-221(r=-0.726,-0.599;both P<0.05).ROC curve analysis showed that the area under the curve of serum miR-21,miR-221 and miR-146 a in diagnosis of PTC was 0.790,0.796,and 0.289,respectively,and the area under the curve of combined them in diagnosis of PTC was 0.914.Conclusion The expression of miR-21 and miRNA-221 in serum of PTC patients is significantly up-regulated,while the expression level of miR-146 a is down-regulated,which can be used as
