摘　　要：目的评价肛门括约肌肌电图(external anal sphincter electromyography,EAS-EMG)卫星电位(satellite poten-tial,SP)对于多系统萎缩(multiple system atrophy,MSA)诊断的意义。方法选取2017年6月至2018年6月首都医科大学宣武医院收治的MSA患者进行EAS-EMG检查,对同期就诊的帕金森病(Parkinson’s disease,PD)患者及招募的健康受试者(healthy control,HC)均进行该检查,获得运动电位(motor unit potential,MUP)各项参数并分析SP形态学参数。结果分别入组30例MSA患者、15例PD患者及20例HC,MSA组的MUP平均时限为(11.8±1.5)ms,比PD组的(10.0±0.6)ms和HC组的(10.1±0.9)ms显著延长,差异有统计学意义(P<0.001)。以SP与主波间距≥1 ms等电位线为标准,MSA组SP出现率为(9.2±7.9)%,显著高于PD的(1.4±2.9)%及HC组的(2.2±2.7)%,差异有统计学意义(P<0.001)。结论SP出现率是MSA电生理诊断的重要参数,但其敏感性与特异性仍有待进一步研究。Objective To study the diagnosis value of satellite potential(SP)in external anal sphincter electromyography(EAS-EMG)for multiple system atrophy(MSA).Methods Patients with MSA,Parkinson’s disease(PD)and healthy controls(HC)were recruited to receive EAS-EMG examination respectively at Xuanwu Hospital of Capital Medical University from June 2017 to June 2018.Motor unit potentials(MUP)were acquired and morphological parameters of SP were analyzed.Results Thirty patients with MSA,15 patients with PD and 20 HC were recruited,respectively.The mean duration of MUP in MSA group[(11.8±1.5)ms]was significantly longer than those in PD group[(10.0±0.6)ms,P<0.001]and HC group[(10.1±0.9)ms,P<0.001].Under the criteria of at least 1 ms interval isopotential line between SP and main wave,SP rate was also significantly higher in MSA group[(9.2±7.9)%]than those in PD group[(1.4±2.9)%,P<0.001]and HC group[(2.2±2.7)%,P<0.001].Conclusions SP can serve as an important parameter in the electrophysiological diagnosis of MSA,but its sensitivity and specificity remain to be clarified.
