外周神经鞘瘤超声特征分析

摘　　要：目的总结外周神经鞘瘤的超声图像的特征。方法回顾性分析2012年1月至2019年12月中国中医科学院望京医院56例经手术后病理证实为外周神经鞘瘤的超声影像资料,总结其声像图特征。结果56例神经鞘瘤中发生于肌层26例,浅筋膜层30例,36例术前超声诊断与术后病理相符,诊断符合率为64.3%。62.5%(35/56)有靶环征,37.5%(21/56)有鼠尾征,30.3%(17/56)有亮帽征,19.6%(11/56)有囊性变,53.6%(30/56)有血流信号显示。肌层神经鞘瘤超声诊断率为84.6%(22/26),明显高于浅筋膜层神经鞘瘤的超声诊断率(14/30,46.7%,P=0.003)。外周神经鞘瘤有靶环征组与无靶环征组、有血流信号组与无血流信号组肿物最大径线比较,差异均有统计学意义(P=0.000,P=0.043);有囊性变组与无囊性变组肿物最大径线比较,差异无统计学意义(P=0.089)。外周神经鞘瘤最大径线与超声表现为有靶环征、有血流信号、有囊性变均有相关性(r=0.497,P=0.000;r=0.277,P=0.038;r=0.317,P=0.017)。结论外周神经鞘瘤具有特征性超声表现,需结合多重表现以提高超声对神经鞘瘤诊断准确率,降低误诊率。Objective To summarize the ultrasound features of peripheral schwannoma.Methods The ultrasound features of 56 cases of schwannoma proved by pathology in Wangjing Hospital of Chinese Academy of Chinese Medical Sciences from January 2012 to December 2019 were retrospectively analyzed.Results Of the 56 schwannomas,26 cases occurred in the muscular layer and 30 cases in the superficial fascia layer.The preoperative ultrasonographic diagnosis of 36 cases was consistent with the postoperative pathology,and the diagnostic coincidence rate was 64.3%.The ultrasonographic findings showed that 62.5%(35/56)had target ring sign,37.5%(21/56)had rat tail sign,30.3%(17/56)had bright cap sign,19.6%(11/56)had cystic change,and 53.6%(30/56)had blood flow signal.The diagnostic rate of muscular schwannoma was 84.6%(22/26),which was significantly higher than that of superficial fascial schwannoma(14/30,46.7%,P=0.003).There was statistically significant difference between the group with and without target ring sign,the group with and without blood flow signal in the largest diameter line of peripheral schwannomas(P=0.000,P=0.043),while there was no statistically significant difference between the group with and without cystic changes in the largest diameter line of tumors(P=0.089).The maximum diameter of peripheral schwannoma was correlated with the ultrasonic apprearance of target ring sign,blood flow signal,and cystic changes in ultrasound(r=0.497,P=0.000;r=0.277,P=0.038;r=0.317,P=0.017).Conclusions Peripheral schwannoma has characteristic ultrasonic appearance,so it is necessary to combine multiple appearances to improve the diagnostic accuracy and reduce the misdiagnosis rate.
