摘  要:目的探讨血清p53抗体、微小RNA-451(miR-451)及癌胚抗原(CEA)在食管鳞状细胞癌诊断中的应用价值。方法选取2019年1月—2020年1月收治的食管鳞状细胞癌89例作为观察组,另选取同期健康体检者89例作为对照组,比较两组血清p53抗体、miR-451及CEA水平,采用Spearman相关性分析探讨食管鳞状细胞癌患者血清p53抗体、miR-451与CEA的相关性;采用受试者工作特征(ROC)曲线分析血清p53抗体、miR-451、CEA及3项联合对食管鳞状细胞癌的诊断价值。结果观察组血清p53抗体、miR-451和CEA水平均高于对照组,差异有统计学意义(P<0.01)。Spearman相关性分析结果显示,食管鳞状细胞癌患者血清p53抗体、miR-451与CEA均呈正相关(P<0.01)。ROC曲线分析结果显示,对食管鳞状细胞癌诊断价值血清p53抗体、miR-451和CEA 3项联合检测略高于单独检测;血清p53抗体、miR-451、CEA及3项联合诊断食管鳞状细胞癌的最佳截断值分别为0.225、1.385、10.830 ng/ml和0.538,此时其敏感度和特异度分别为96.6%、84.3%,60.7%、100.0%,50.6%、100.0%和89.9%、88.8%。结论食管鳞状细胞癌患者血清p53抗体、miR-451和CEA水平升高,血清p53抗体、miR-451与CEA均呈正相关,对食管鳞状细胞癌诊断价值血清p53抗体、miR-451和CEA 3项联合检测略高于单独检测。Objective To explore application value of serum p53 antibody,micro RNA-451(miR-451)and carcinoembryonic antigen(CEA)in diagnosis of patients with esophageal squamous cell carcinoma(ESCC).Methods A total of 89 patients with ESCC admitted during January 2019 and January 2020 were selected as observation group,while another 89 healthy people undergoing physical check-ups at the same period were selected as control group.Serum levels of p53 antibody,miR-451 and CEA were compared between two groups.Spearman correlation analysis was used to analyze correlations among serum p53 antibody,miR-451 and CEA in patients with ESCC.Receiver operating characteristics(ROC)curve was used to analyze values of serum p53 antibody,miR-451,CEA and combination of the three items in diagnosis of ESCC.Results Serum levels of p53 antibody,miR-451 and CEA in observation group were significantly higher than those in control group(P<0.01).Spearman correlation analysis showed that serum p53 antibody and miR-451 were positively correlated with CEA in patients with ESCC(P<0.01).Results of ROC curve analysis showed that value of the combination of the three items in diagnosis of ESCC was slightly higher than those by the three items tests alone.The optimal cut-off values of serum p53 antibody,miR-451,CEA and the combination of the three in diagnosis of ESCC were 0.225,1.385,10.830 ng/ml and 0.538,and their corresponding values of sensitivity and specificity were 96.6%,84.3%,60.7%,100.0%and 50.6%,100.0%,89.9%,88.8%,respectively at that moment.Conclusion levels of serum p53 antibody,miR-451 and CEA are elevated in patients with ESCC.Serum p53 antibody and miR-451 are positively correlated with CEA,and the diagnostic value of combined test of serum p53 antibody,miR-451 and CEA is slightly higher than those by the three items tests alone.
