摘　　要：目的探讨计算机辅助实时导航技术在颌面部额外牙取出术中应用及推广的可能性。方法本次研究收录了上海交通大学医学院附属第九人民医院耳鼻咽喉头颈外科于2014年11月至2020年8月收治的5例颌面部额外牙的患者。其中,男性4例,女性1例。鼻底部额外牙3例、鼻窦额外牙1例,下颌骨升支额外牙1例。所有病例均术前行颌面部CT,根据术前CT资料设计个体化导航手术方案,并全麻实时导航辅助下完成内镜下颌面部额外牙取出术。结果所有患者术中均未伤及重要血管及神经,额外牙完整并安全取出。术后随访所有患者均未出现手术并发症,额外牙无残留。结论对于颌面部额外牙的患者,术前设计个体化手术方案、术中实时导航有利于快速精准地找到额外牙并完整安全地取出,同时可避开周围重要的血管神经,避免近期及远期手术并发症。Objective To explore the possibility of application and popularization of computer-assisted real-time navigation technology in the extraction of maxillofacial extra teeth extraction.Methods From November 2014 to August 2020,5 patients with maxillofacial extra teeth were enrolled in this study.There were 4 males and 1 female,3 extra teeth in the bottom of nose,1 extra teeth in the sinuses and 1 mandibular ramus extra teeth.Preoperative maxillofacial CT was performed in all the cases.Individualized navigation surgery was designed according to preoperative CT data,and endoscopic additional tooth extraction was completed with the assistance of real-time navigation under general anesthesia.Result TThe extra teeth were removed safely and completely without injury of important blood vessels or nerves.Postoperative 3-month follow-up revealed neither surgical complications nor residual additional teeth in all patients.Conclusion For patients with extra teeth in maxillofacial region,the design of individualized surgical plan and intraoperative real-time navigation is conducive to the rapid and accurate finding of extra teeth and the complete and safe extraction while avoiding surgical complications due to injury of important blood vessels or nerves.
