摘　　要：目的观察超声造影(CEUS)联合声触诊组织成像定量(VTIQ)技术诊断乳腺癌前哨淋巴结(SLN)的价值。方法对37例经病理确诊乳腺癌患者行乳腺超声造影,追踪显像淋巴管,观察SLN增强模式,评价CEUS对SLN的检出率及定位及定性诊断准确率;采用VTIQ测量CEUS所见SLN的剪切波速度平均值(SWV mean)。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),判断VTIQ诊断SLN的效能;观察2种方法联合诊断SLN的价值。结果CEUS对乳腺癌SLN的检出率为91.89%,判断其良恶性的敏感度为90.91%,特异度为51.72%,准确率为62.50%。SWV mean诊断SLN的AUC为0.848,截断值取1.79 m/s时,敏感度、特异度及准确率分别为90.91%、68.97%及75.00%。CEUS和VTIQ联合诊断SLN良恶性淋巴结的敏感度、特异度及准确率为90.91%、75.86%及80.00%。结论CEUS及VTIQ联合诊断乳腺癌SLN的效能较高。Objective To observe the value of contrast enhanced ultrasound(CEUS)combined with virtual touch tissue image quantification(VTIQ)for diagnosing sentinel lymph nodes(SLN)of breast cancer.Methods A total of 37 patients with pathologically proved breast cancer were enrolled.The lymphatic vessels were traced and the enhancement patterns of SLN were observed,and the detection rate,localization and qualitative accuracy of SLN were evaluated during CEUS.The mean shear wave velocity(SWV mean)of SLN marked by CEUS was measured with VTIQ.The receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)was calculated to judge the effectiveness of VTIQ in diagnosis of SLN.The value of CEUS combined with VTIQ in diagnosing SLN was analyzed.Results The detection rate of CEUS for SLN of breast cancer was 91.89%,the sensitivity of diagnosis of benign and malignant was 90.91%,and the specificity was 51.72%,the accuracy was 62.50%.AUC of SWV mean for diagnosing SLN was 0.848.Taken 1.79 m/s as the cutoff value of SWV mean,the sensitivity,specificity and accuracy of VTIQ for judging benign and malignant SLN was 90.91%,68.97%and 75.00%,respectively.The sensitivity,specificity and accuracy of combining CEUS and VTIQ in diagnosing benign and malignant SLN was 90.91%,75.86%and 80.00%,respectively.Conclusion Combination of CEUS and VTIQ had high efficacy for diagnosing SLN of breast cancer.
