磁共振全身弥散加权成像联合常规CT/MRI在肺癌转移灶诊断中的应用

摘　　要：目的:磁共振全身弥散加权成像(WB-DWI)联合常规CT/MRI在肺癌转移灶诊断中的应用价值。方法:选取2018年3月—2020年3月于河南宏力医院接受治疗的73例疑似肺癌转移患者作为研究对象,所有患者均行WB-DWI及常规CT/MRI检查,以病理结果作为“金标准”,分析WB-DWI联合常规CT/MRI在肺癌转移灶中的诊断价值。结果:73例患者经病理结果证实,61例患者发生转移,其中淋巴结转移26例,占比42.62%;骨转移23例,占比37.70%;其他脏器转移12例,占比19.67%;以病理结果为“金标准”,WB-DWI检查对肺癌转移灶诊断具有理想的一致性(Kappa=0.695);常规CT/MRI检查对肺癌转移灶诊断具有理想的一致性(Kappa=0.591);WB-DWI联合常规CT/MRI检查对肺癌转移灶诊断具有极好的一致性(Kappa=0.795)。结论:与病理结果相比,WB-DWI联合常规CT/MRI检查对肺癌转移灶的诊断具有极高的一致性,可为临床诊断与治疗肺癌转移灶提供重要依据,值得广泛推广与应用。Objective:To evaluate the value of whole-body diffusion weighted imaging(WB-DWI)combined with conventional CT/MRI in the diagnosis of lung cancer metastasis.Methods:73 patients with suspected lung cancer metastasis who were treated in the hospital from March 2018 to March 2020 were selected as the research objects.All patients underwent WB-DWI and conventional CT/MRI examination.The pathological results were taken as the"gold standard" to analyze the diagnostic value of WB-DWI combined with conventional CT/MRI in lung cancer metastasis.Results:Among 73 patients,61 cases had metastasis,including 26 cases of lymph node metastasis(42.62%),23 cases of bone metastasis(37.70%),12 cases of other organ metastasis(19.67%).The pathological results were taken as the"gold standard",WB-DWI examination had ideal consistency in the diagnosis of lung cancer metastasis(Kappa=0.695).Conventional CT/MRI examination had ideal consistency in the diagnosis of lung cancer metastasis(Kappa=0.591).WB-DWI combined with conventional CT/MRI had excellent consistency(Kappa=0.795).Conclusion:Compared with the pathological results,WB-DWI combined with conventional CT/MRI has high consistency in the diagnosis of lung cancer metastasis,which can provide important basis for clinical diagnosis and treatment of lung cancer metastasis,which is worthy of extensive promotion and application.
