氟哌噻吨美利曲辛治疗非糜烂性胃食管反流病疗效的Meta分析

摘　　要：目的系统分析氟哌噻吨美利曲辛片联合质子泵抑制剂(proton pump inhibitor,PPI)疗法治疗非糜烂性胃食管反流病(non-erosive gastroesophageal reflux disease,NERD)的有效性及安全性。方法通过计算机检索PubMed、Embase、Cochrane Library、CBM、CNKI、维普、万方等数据库,收集氟哌噻吨美利曲辛片治疗NERD患者的调查研究。采用Stata 12.0、Revman 5.1软件进行Meta分析。结果最终纳入23篇文献。纳入研究对象2242例,其中观察组(氟哌噻吨美利曲辛片联合PPI)1125例,对照组(PPI常规治疗)1117例。Meta分析显示,观察组治疗NERD的有效性优于对照组(RR=1.26,95%CI:1.21~1.32,P<0.0001);不良反应的RR值为1.13(95%CI:0.82~1.55,P=0.463),其安全性与对照组比较,差异无统计学意义;观察组与对照组相比,可显著降低患者焦虑评分(SMD=-1.77,95%CI:-2.51~-1.03,P<0.0001)和抑郁评分(SMD=-1.90,95%CI:-2.64~-1.17,P<0.0001)。结论氟哌噻吨美利曲辛联合PPI疗法可显著改善患者NERD及焦虑抑郁症状,安全性与常规疗法相比无差异。Objective To evaluate the efficacy and safety of Flupenthixol and melitracen tablets combined with proton pump inhibitor(PPI)therapy in the treatment of non-erosive gastroesophageal reflux disease(NERD).Methods Studys on the treatments of NERD patients with Flupenthixol and melitracen tablets were collected through searching PubMed,Embase,Cochrane Library,CBM,CNKI,VIP and WanFang databases.Meta-analysis was performed by using Stata 12.0 and Revman 5.1 software.Results A total of 23 articles and 2242 subjects were included.1125 subjects were treated with Flupenthixol and melitracen tablets combined with PPI therapy in the observation group and 1117 subjects were treated with PPI in the control group.The analysis showed that in the observation group,the RR value of the efficacy was 1.26(95%CI:1.21-1.32,P<0.0001),which was more effective than that in the control group.The RR value of adverse reactions was 1.13(95%CI:0.82-1.55,P=0.463),which was no significant defference from that in the control group.Compared with the control group,the observation group significantly reduced the patients’anxiety scores(SMD=-1.77,95%CI:-2.51--1.03,P<0.0001)and depression scores(SMD=-1.90,95%CI:-2.64--1.17,P<0.0001).Conclusion Compared with the conventional therapy,the Flupenthixol and melitracen tablets combined with PPI therapy significantly improve the symptoms of NERD and reduce the anxiety and depression scores,the safety of which is no difference from conventional therapy.
