调中益气汤加味治疗脾虚湿滞证功能性消化不良的临床疗效观察

摘　　要：目的:探讨调中益气汤治疗功能性消化不良患者的临床疗效及对患者焦虑抑郁状态和生活质量的影响。方法:将90例脾虚湿滞证功能性消化不良患者随机分为观察组和对照组,每组45例。观察组予调中益气汤加味治疗,对照组予多潘立酮片治疗,疗程均4周,随访3个月。观察两组的临床疗效和复发率差异,比较两组患者SF-36、HAMA、HAMD-17评分。结果:治疗4周后,观察组的愈显率和显效率明显高于对照组(P<0.05),观察组的复发率明显低于对照组(P<0.05);观察组和对照组在治疗第4周末及随访3个月末的HAMA评分及HAMD-17评分与治疗前相比均明显降低(P<0.01或P<0.05),且观察组的HAMA评分及HAMD-17评分与对照组同期的相比降低更显著(P<0.01);观察组在治疗第4周末及随访3个月末的SF-36评分与治疗前相比均明显升高(P<0.01),对照组在治疗第4周末的SF-36评分高于治疗前(P<0.05),且观察组的SF-36评分与对照组同期的相比升高更显著(P<0.05)。结论:调中益气汤加味治疗功能性消化不良,可提高愈显率和显效率,降低复发率,改善患者的焦虑抑郁状态,提高生活质量,值得在临床上推广。Objective:To investigate the clinical efficacy of Tiaozhong Yiqi decoction in the treatment of patients with functional dyspepsia and its effect on anxiety,depression and quality of life.Methods:90 patients with functional dyspepsia with dampness stagnation syndrome of spleen deficiency were randomly divided into observation group and control group,45 patients in each group.The observation group was treated with Tiaozhong Yiqi decoction,and the control group was treated with domperidone tablet.The course of treatment was 4 weeks,and the patients were followed up for 3 months.The clinical efficacy and recurrence rate of the two groups were observed.The scores of SF-36,Ham-14 and HAMD-17 were compared between the two groups.Results:After 4 weeks of treatment,the curative and significant efficiency rate of the observation group were significantly higher than that of the control group(P<0.05),the recurrence rate in the observation group was significantly lower than that in the control group(P<0.05);the HAMA score and HAMD-17 score of the observation group and the control group at the end of the 4 th week of treatment and the end of the 3 rd month of follow-up were significantly lower than those before treatment(P<0.01 or P<0.05);and the HAMA score and HAMD-17 score of the observation group were significantly lower than those of the control group(P<0.01);the SF-36 score were significantly higher in observation group at the end of the 4 th week of treatment and at the end of the 3 rd month of follow-up than that of before treatment(P<0.01),the SF-36 score in the control group was increased significantly than that of before treatment(P<0.05),and the and the SF-36 score of the observation group increased more significantly than that of the control group(P<0.05).Conclusion:Tiaozhong Yiqi decoction in the treatment of functional dyspepsia can improve the markedly effective rate,reduce the recurrence rate,improve the anxiety and depression of patients,and improve the quality of life,which is worthy of clinical promotion.
