摘　　要：目的探究胃癌患者幽门螺杆菌(Hp)感染与胃癌组织c-myc、增殖细胞核抗原(PCNA)的关系及其对预后的影响。方法收集2013年6月-2015年6月在南阳医学高等专科学校第一附属医院普通外科接受手术治疗的100例不同胃黏膜病变患者和34例胃癌(GC)患者的病理组织标本,其中浅表性胃炎(CSG)13例,萎缩肠化生胃炎(CAG+IM)26例,异型增生(DYS)27例,早期胃癌(EGC)10例,进展期胃癌(AGC)24例。胃镜取材快速尿素酶试验及病理组织切片Giemsa染色检测Hp感染,采用免疫组织化学法检测不同病变组织的c-myc、PCNA表达。结果CAG+IM、DYS、GC患者Hp阳性率均高于CSG患者(P<0.05);GC患者中,AGC患者Hp阳性率均高于EGC患者(P<0.05);CAG+IM、DYS、GC患者c-myc蛋白、PCNA蛋白表达均高于CSG患者(P<0.05);c-myc蛋白表达与胃癌浆膜浸润和淋巴结转移有关(P<0.05),PCNA蛋白表达与胃癌浆膜浸润和肝转移有关(P<0.05);肠型胃癌Hp感染患者c-myc阳性表达率较弥漫型高(P<0.05)。胃癌Hp感染者5年生存率12.00%低于非Hp感染者55.56%(P<0.05)。结论胃癌患者Hp感染和c-myc、PCNA表达与胃黏膜增殖和恶化有关,Hp感染也与胃癌患者预后有关。OBJECTIVE To investigate the relationship between Helicobacter pylori(Hp)infection and c-myc and proliferating cell nuclear antigen(PCNA)in gastric cancer tissues and the influence on prognosis in patients with gastric cancer.METHODS The pathological tissue specimens were collected from 100 patients with different gastric mucosal lesions and 34 patients with gastric cancer(GC)who underwent surgical treatment in the First Affiliated Hospital of Nanyang Medical College between Jun 2013 and Jun 2015.They included 13 cases of chronic superficial gastritis(CSG),26 cases of atrophic gastritis with intestinal metaplasia(CAG+IM),27 cases of dysplasia(DYS),10 cases of early gastric cancer(EGC)and 24 cases of advanced gastric cancer(AGC).Gastroscopy biopsy specimens were used to detect Hp infection by rapid urea enzyme test and Giemsa staining.Immunohistochemical method was used to detect the expression of c-myc and PCNA in different disease tissues.RESULTS The Hp positive rate in patients with CAG+IM,DYS and GC was significantly higher than those with CSG(P<0.05).In patients with GC,the Hp positive rate in patients with AGC was significantly higher than those with EGC(P<0.05).The expression of c-Myc and PCNA protein was significantly higher in patients with CAG+IM,DYS and GC than that with CSG(P<0.05).The expression of c-Myc protein was related to serous infiltration and lymph node metastasis of gastric cancer(P<0.05),and the expression of PCNA protein was related to serosal infiltration and liver metastasis of gastric cancer(P<0.05).The positive expression rate of c-Myc in gastric cancer patients with intestinal Hp infection was significantly higher than that of diffuse type(P<0.05).The 5-year survival rate of the gastric cancer patients with Hp infection was 12.00%,which was significantly lower than 55.56%of those without Hp infection(P<0.05).CONCLUSION Hp infection and the expression of c-Myc and PCNA in patients with gastric cancer are related to proliferation and deterioration of gastric mucosa.Hp infection is also r
