摘  要:目的分析高危型人乳头瘤状病毒(HPV)感染对乳腺癌患者癌组织中核因子κB (NF-κB)p50、NF-κB p65及凋亡相关指标的影响。方法选取2017年8月-2020年7月于余姚市人民医院行乳腺癌手术治疗的150例乳腺浸润性癌患者为研究对象,采用第二代捕获杂交法检测高危型HPV感染情况和病毒载量,免疫组织化学法检测癌组织和癌旁组织NF-κB p50、NF-κB p65蛋白表达,实时荧光定量PCR检测Bcl-2、Bax和Caspase-3 mRNA水平。结果 150例乳腺癌患者HPV感染95例,其中HPV 16型感染52例,18型感染35例,33型感染8例,HPV DNA载量为(2.08±0.50)lg(RLU/CO);HPV感染与乳腺癌组织学分级、ER阴性、PR阴性和淋巴结转移有关(P<0.05);乳腺癌患者癌组织中NF-κB p50和NF-κB p65阳性表达率均高于癌旁组织(P<0.05),HPV感染组患者癌组织中NF-κB p50、NF-κB p65阳性表达率分别为66.32%、70.53%,均高于未感染组(P<0.05);乳腺癌患者癌组织中Bcl-2 mRNA水平高于癌旁组织(P<0.05),Bax和Caspase-3 mRNA水平均低于癌旁组织(P<0.05);HPV感染组癌组织中Bcl-2 mRNA水平(0.62±0.11)高于未感染组(t=7.207,P<0.001),Bax和Caspase-3 mRNA水平分别为(0.29±0.05)和(0.27±0.05)均低于未感染组(t=11.383,7.881;P均<0.001)。结论高危型HPV感染可以激活乳腺癌患者癌组织中NF-κB p50、NF-κB p65的表达,抑制凋亡基因表达。OBJECTIVE To analyze the effects of high-risk human papilloma virus(HPV) infection on nuclear factor-κB(NF-κB) p50, NF-κB p65 and apoptosis-related indexes in breast cancer tissues. METHODS A total of 150 patients with invasive breast cancer who underwent breast cancer surgery in the hospital from Aug 2017 to Jul 2020 were enrolled in the study.The infection status and viral load of high-risk HPV were detected by second-generation capture hybridization test.The expression of NF-κB p50 and NF-κB p65 proteins in cancer tissues and para-cancerous tissues was detected by immunohistochemistry.The levels of Bcl-2, Bax and Caspase-3 mRNA were detected by real-time fluorescent quantitative PCR. RESULTS There were 95 cases of HPV infection in 150 breast cancer patients, including 52 cases of HPV type 16 infection, 35 cases of HPV type 18 infection, 8 cases of HPV type 33 infection.The HPV DNA load was(2.08±0.50) lg(RLU/CO).HPV infection rate was related to histological grade of breast cancer, ER negative, PR negative and lymph node metastasis(P<0.05).The positive expression rates of NF-κB p50 and NF-κB p65, and level of Bcl-2 mRNA in cancer tissues were significantly higher than those in para-cancerous tissues(P<0.05), while levels of Bax and Caspase-3 mRNA were significantly lower than those in the para-cancerous tissues(P<0.05). The positive expression rates of NF-κB p50 and NF-κB p65 were 66.32% and 70.53%, which were significantly higher than those in the non-infection group.The level of Bcl-2 mRNA in HPV infection group(0.62±0.11) were significantly higher than that in non-infection group(t=7.207,P<0.001), while levels of Bax and Caspase-3 mRNA were(0.29±0.05) and(0.27±0.05), respectively, which were significantly lower than those in non-infection group(t=11.383, 7.881;all P<0.001). CONCLUSION High-risk HPV infection can activate the expression of NF-κB p50 and NF-κB p65 in breast cancer tissues, which can inhibit the expression of apoptosis genes.
